successful completion of training by the person.
The results will be recorded on the appropriate Certification/
Qualification Examination Report (MSHA Form 5000-17) by the
Agency's representative who conducted the examination and
forwarded to the Qualification and Certification Unit, P.O. Box
25367, Denver, CO 80225-0367.
The qualification status will become a permanent record in the
Agency's data base and will not expire as long as retraining is
attended on an annual basis.
