<strong>Golden</strong> <strong>Nematode</strong> Farm Survey Questionnaire<br />
Purpose<br />
The <strong>Golden</strong> <strong>Nematode</strong> Farm Survey Questionnaire is used to<br />
document why the survey will take place, how many acres are in the<br />
survey, and how many samples to expect during the interview with the<br />
farmer or grower. The form is also for PPQ Officers to keep a record of<br />
the entire crop production per year, per grower.<br />
Instructions<br />
Complete the <strong>Golden</strong> <strong>Nematode</strong> Farm Survey Questionnaire as shown<br />
in Table A-1-3. The information from the completed questionnaire is<br />
then transferred to the Laboratory Leader removes the completed<br />
sheets weekly and files in the <strong>Golden</strong> <strong>Nematode</strong> File Folder in<br />
the Work Unit files..<br />

1 COUNTY LIST the name of the county where the land for survey<br />
is located<br />
2 YEAR LIST the year the survey will be conducted (If a<br />
confirmation survey, then interview could take place in<br />
the fall of one year, but the survey won’t be conducted<br />
until the next year)<br />
3 GROWER LIST the name of the grower or the farm<br />
4 DATE LIST the date the interview is being conducted<br />
5 TOTAL ACRES IN POTATOES LIST the total number of acres planted to potatoes (all<br />
varieties)<br />
6 TOTAL ACRES IN RESISTANT<br />
VARIETY POTATOES<br />
LIST the total number of acres planted to resistant<br />
varieties (RV) of potatoes<br />
7 TOTAL ACRES TO SURVEY SUBTRACT the number listed in Block 6 from the<br />
number listed in Block 5 and ENTER the difference in<br />
this block<br />
10 SURVEY METHOD PLACE an X in the appropriate block<br />
