2. Applicants who are less than 18 years of age must obtain parental<br />

or legal guardian consent on DD Form 373 (Consent Declaration of Parent<br />

or Legal Guardian) prior to enlistment in the MCR. This form will be<br />

prepared in triplicate.<br />

3. The correct age of all applicants will be verified by the OSO and a<br />

copy of one of the following will be forwarded in the application:<br />

a. Certificate of birth.<br />

b. Age as given on passports.<br />

c. Date of birth as shown on last Discharge Certificate or DD Form<br />

214, Armed Forces Report of Transfer or Separation.<br />

d. DD Form 372, Application for Verification of Birth completed by<br />

either the State, County, or City Bureau of Vital Statistics or Department<br />

of Public Health (Figure 2-17).<br />

e. NAVMC 5002, Age Certificate, which shall be used only when<br />

supported by one of the following:<br />

(1) A transcript of records of the grammar school attended,<br />

evidencing the applicant’s date of birth.<br />

(2) A transcript of records of the hospital in which the<br />

applicant was born evidencing the applicant’s date of birth.<br />

f. Baptismal certificate showing date of birth.<br />

4. Driver’s licenses and/or college transcripts do not qualify as<br />

birth verification.<br />

2206. DEPENDENCY AND MARITAL REQUIREMENTS<br />

1. Provided they are otherwise qualified, PLC, OCC, and WOC applicants<br />

with more than one dependent or if a single parent, may submit an<br />

application requesting that a dependency waiver be granted by the CMC.<br />

For this purpose, a spouse is considered a dependent.<br />

2. A definition of "dependency" is provided to determine dependency<br />

status for enlistment purposes only. The definition does not apply to<br />

entitlements of pay and allowances, medical care, exchange and<br />

commissary privileges, or other benefits. Such determinations are made<br />

only after enlistment, and recruiting personnel will not render<br />

opinions on future entitlements. A dependent for enlistment purposes<br />

is defined as:<br />

a. A spouse to include a common law spouse if the state recognizes<br />

such; or<br />

b. Any natural child (legitimate or illegitimate) or child adopted<br />

by the applicant, if the child is under 18 years of age and unmarried,<br />

regardless of whether or not the applicant has custody of the child.<br />

For male applicants, the term "natural child" includes any illegitimate<br />

child when the applicant claims the child as his, or the applicant’s<br />

name is listed on the birth certificate as the father, or a court<br />

order establishes paternity; or<br />

c. A stepchild of the applicant who resides with the applicant if<br />

the stepchild is under 18 years of age; or<br />

d. Any parent or other persons who are, in fact, dependent on the<br />

applicant for over one half of their support.<br />

e. If an applicant’s dependent child has been formally adopted by<br />

another person (final adoption decree issued and effective), then the<br />

child is not the applicant’s dependent.<br />

3. In all cases requiring a dependency waiver, the OSO will ensure<br />

that all parties fully understand the probability of unaccompanied<br />

overseas tours of duty.<br />
