CHAPTER 2<br />

MARINE CORPS RESERVE OFFICER CANDIDATE PROGRAMS<br />

SECTION 3: SOURCE, PREPARATION, AND SUBMISSION OF FORMS/DOCUMENTS/<br />

INFORMATION<br />

2300. SUPPLY OF BLANK FORMS. All NAVMC, DD, FD, and SF forms will be<br />

requisitioned through normal supply channels. Others may be reproduced<br />

locally.<br />

2301. PREPARATION OF FORMS DOCUMENTS. A record book will be assembled<br />

for each individual who is conditionally enlisted in the MCR for<br />

enrollment in an officer candidate program. OSO’s will verify each<br />

applicant’s SSN prior to administrative processing. The application<br />

will be prepared and assembled in a six section officer candidate<br />

folder, to include the Officer Candidate Application (OCA), an Officer<br />

Qualification Record (OQR), and the HR.<br />

2302. THE OFFICER CANDIDATE APPLICATION<br />

1. Section 1 General Correspondence<br />

a. Copy of Physical Transmittal. A copy of the transmittal<br />

forwarding the original physical from the District to its appropriate<br />

destination (paragraph 2202) will be the top document in this section.<br />

In cases where the physical cannot be found, this will assist in<br />

tracing procedures.<br />

b. Copy of Physical. Copies of both the SF 88 and SF 93 will be<br />

included along with copies of every consultation. Cycloplegic eye<br />

examinations and anthropometric body measurements for aviation applicants<br />

will also be included.<br />

c. Chapter 15 of the Manual of the Medical Department contains<br />

instructions for the preparation of physical examinations.<br />

d. Also include the Dental Statement and the Privacy Act Statement<br />

for HR as the bottom two enclosures.<br />

2. Section 2 Orders. NAVMC 10468 (Rev 7-83) Request for Orders<br />

(Figure 2-3)<br />

a. Submit the original only for all applications. Entries will be<br />

typewritten or printed clearly in black ink. ZIP codes must be<br />

included with all addresses.<br />

b. For PLC applicants, the increment of training requested on the<br />

Request for Orders must match the increment requested in ARMS and on<br />

the NAVMC 10418-1 Application for Officer Programs.<br />

c. For active PLC’s scheduled to attend the senior course of<br />

training, requests for orders must arrive at the CMC (MRRO-2) by 31<br />

January of the calendar year of the scheduled training.<br />

3. Section 3 Financial Assistance, FAP, and FIP. Leave empty.<br />

4. Section 4 National Agency Check<br />

a. DD Form 4 (Rev 5-85) Enlistment Reenlistment Document (Figure<br />

2-4)<br />

(1) All provisions of the DD Form 4, commonly termed the<br />

enlistment contract, will be thoroughly explained to applicants so that<br />

they fully understand that the enlistment is conditional and not final<br />

until approved by the CMC.<br />

(2) Normal use of capital letters and spacing will be used.<br />

The document must be typed using a ribbon that will produce black or<br />

blue-black impressions capable of marking each page of the document<br />

suitable for reproduction by photographic or other duplicating<br />

processes. All copies must be legible. Signatures must be in black or<br />
