6. Section 6 Application<br />

a. Copy of ARMS Screen I. A copy of the verified ARMS Screen I<br />

for each individual candidate will be reproduced and placed as the<br />

first document in this section.<br />

b. NAVMC 10418-1 (Rev. 7-86) Application for Officer Programs<br />

(Figure 2-22)<br />

(1) The original of this form will be submitted with applications<br />

for all officer candidate programs.<br />

(2) This form will be filled out utilizing current instructions<br />

as contained in figure 2-22 of this Manual.<br />

(3) OSO’s are advised to counsel applicants to truthfully<br />

respond to items 1 through 16. The applicant will place initials in<br />

the block corresponding to response for each question.<br />

(4) A "yes" answer to any question on the Application for<br />

Officer Programs will require additional information. The OSO will<br />

thoroughly investigate the circumstances surrounding the affirmative<br />

answer to verify the applicant provided information. This verification<br />

should take the form of completion of the DD Form 369, Police Record<br />

Check, interview with the college dean, or any other methods deemed<br />

appropriate by the district director and/or OSO. The information<br />

obtained should be included with the application.<br />

(5) The OSO will ensure that the applicant understands that any<br />

arrest, regardless of severity, expungement, etc., must be explained.<br />

Paragraph 2209 of this Manual explains the various terms used in the<br />

legal system.<br />

(6) Since the NAVMC 10418-1 is the source document for ARMS,<br />

the OSO will verify the information contained on it to ensure the<br />

accuracy of the data base.<br />

c. DD Form 785 Record of Disenrollment from Officer Candidate Type<br />

Training (Figure 2-23)<br />

(1) The original, completed DD Form 785 will be submitted for<br />

all applicants from a civilian source who have previously disenrolled<br />

from any U.S. Armed Forces officer candidate program other than a MCR<br />

Officer Program. A statement from the applicant stating the reason for<br />

disenrollment will be attached to the DD Form 785 and submitted with<br />

the application.<br />

(2) DD Form 785 is not required for an applicant who has<br />

successfully completed the 2-year level of basic nonscholarship ROTC,<br />

provided the applicant has not executed a contract for advanced ROTC.<br />

In lieu of the DD Form 785 in this case, the word BASIC would be<br />

written in block 1 of the NAVMC 10418-1.<br />

(3) DD Form 785 is required for applicants who participated in<br />

any ROTC scholarship program. A statement from the applicant stating<br />

the reasons for scholarship termination will be attached to the DD Form<br />

785 and submitted with the application.<br />

(4) OSO’s should request completion of the DD Form 785 from the<br />

following sources:<br />

(a) For Disenrolled Army ROTC Cadets. Professor of<br />

Military Science (college attended).<br />

(b) For Disenrolled Air Force Cadets. Professor of<br />

Aerospace Studies (college attended).<br />

(c) For Disenrolled NROTC Cadets. Professor of Naval<br />

Science (college attended).<br />
