19b Name Type or print the name of the officer<br />
who administered the oath of<br />
enlistment/reenlistment.<br />
19c Pay Grade Type or print the grade of the officer<br />
(e.g., Capt O-3).<br />
19d Unit/Command Name Use the name of the unit to which the<br />
officer who administered the oath is<br />
attached (e.g., USMC OSS NASHVILLE).<br />
19e Signature The officer who administered the oath<br />
of enlistment/reenlistment and is<br />
identified in block 19b will sign with<br />
initials and last name.<br />
19f Date Signed Enter the date in the year, month, day<br />
formats. Date may be handwritten,<br />
typed, or stamped.<br />
19g Unit/Command Address Enter the city, state, and ZIP of<br />
the unit identified in block 19d<br />
(e.g., NASHVILLE, TN 37203).<br />

(4) The OSO will ensure that applicants are contracted only<br />
after they have qualified on one of the mental tests, passed a physical<br />
examination, and are otherwise qualified for enrollment into a Reserve<br />
Officer Program. The OSO will also ensure that each applicant is sworn<br />
in, by an officer, at an appropriate ceremony at the time of enlistment<br />
(paragraph 2200.4). Applicants currently serving in other branches of<br />
the Armed Forces will not be contracted prior to receipt of an approved<br />
conditional release from the member’s branch.<br />
(5) The copies of the DD Form 4 will be distributed as follows:<br />
Originals on the right side of the OQR, the yellow copy on the left<br />
side of the OQR, the green copy in section 4 of the OCA folder. The<br />
remaining copies will be maintained by the OSO. Applicants currently<br />
serving in the USMCR will not execute new contracts, but copies of<br />
their current Reserve contracts will be substituted in the appropriate<br />
place in the application.<br />
(6) Applicants will be contracted at such time to allow for<br />
approval by CMC within 90 days. The CMC reserves the right to request<br />
new contracts when the delay incurred in processing the application is<br />
deemed excessive.<br />
(7) Contracts will only be executed on those applicants for<br />
which approval for quota is desired.<br />
(8) Two annexes to the basic document are mandatory. Annex B<br />
is the Statement of Conditional Enlistment (Figure 2-5). Annex C is<br />
the appropriate service agreement. These will be placed immediately<br />
