The previous nine examples are the most popular process data collection activities. You may have other, creative,<br />

process data collection methods, so please help us appreciate them by describing them in the text box<br />

shown in Figure 37. For example this form should be completed if you have a website as part <strong>of</strong> your intervention.<br />

In this case, you should also describe how you will track and monitor website usage. Please notice that<br />

the process data collection activity should be related to evaluation. In other words, look at all the activities<br />

that will help you conduct evaluation, monitor the progress <strong>of</strong> the program, or provide information for you to<br />

modify and improve your program.<br />
