Sample <strong>Evaluation</strong> Plans<br />
Enclosed is the <strong>OTIS</strong> <strong>Evaluation</strong> <strong>Guide</strong> CD <strong>of</strong> sample evaluation plans. In this CD, you will find 10 files <strong>of</strong> sample<br />
evaluation plans – one for each <strong>of</strong> the nine Communities <strong>of</strong> Excellence core indicators and one asset. The<br />
sample evaluation plans should only be used as a guide when creating your plans. The evaluation designs or<br />
evaluation activities that you have (or the manner in which they are conducted) may differ. The difference will<br />
depend on how much you want to be able to say about the impact <strong>of</strong> your interventions, your resources, your<br />
target population and/or the characteristics <strong>of</strong> your jurisdiction.<br />
Each sample plan file consists <strong>of</strong> the following components:<br />
1. Cover sheet:<br />
a. Indicator number and description <strong>of</strong> the indicator<br />
b. Sample objective<br />
c. Plan type.<br />
2. <strong>OTIS</strong> summary screens for the plan for the following sections:<br />
a. <strong>Evaluation</strong> Design<br />
i. Scope <strong>of</strong> Work/Objective View<br />
b. <strong>Evaluation</strong> Activity Plan<br />
i. Outcome Data Collection and/or Process Data Collection, depending on the type <strong>of</strong> objective<br />
ii. <strong>Evaluation</strong> Reporting<br />
c. <strong>Evaluation</strong> Summary Narrative<br />
This component follows guidelines outlined in Chapter 4, part D. The evaluation summary narrative<br />
provides the rationale for the major components <strong>of</strong> your evaluation plan and embellishes details<br />
about the evaluation design and activities that are not covered in the <strong>OTIS</strong> evaluation questions. This<br />
section is different from the narrative summaries for 1) Community Assessment Analysis, 2) Major<br />
Intervention Activities and 3) Theory <strong>of</strong> Change.<br />
Please note that the <strong>Guide</strong> was designed to help you answer the evaluation questions in <strong>OTIS</strong> only. If you<br />
