Editorial, Vesalius, VI, 2, 82, 2000EditorialThis winter edition <strong>of</strong> our <strong>journal</strong> follows <strong>the</strong> lines established by Dr John Cule, its outstanding editor. The <strong>journal</strong>continues to expand, although certain factors still limit <strong>the</strong> inclusion <strong>of</strong> written material and author range.The 2000 <strong>International</strong> Conference, held in <strong>the</strong> New World, was a step on <strong>the</strong> way to wi<strong>de</strong>ning this range andinterest, which will have to continue if its rea<strong>de</strong>rship is to extend and allow it become a truly world-wi<strong>de</strong> interestpublication. At Galveston, what could be called 'more mo<strong>de</strong>rn' aspects <strong>of</strong> <strong>the</strong> history <strong>of</strong> medicine were givenprominence. The difficulties in South and North American continents in what is presently styled 'provision <strong>of</strong> healthcare' were well <strong>de</strong>scribed and analysed. The entry <strong>of</strong> politics into medicine was well documented. The increasingpower <strong>of</strong> <strong>the</strong> <strong>la</strong>y administrator, who has become <strong>the</strong> tyrannical figure some doctors with a historical perspectivewarned <strong>of</strong>, now turns health care into arrangements which are ma<strong>de</strong> to save money.Those <strong>of</strong> us who actively teach medical stu<strong>de</strong>nts know well that we have to persua<strong>de</strong> <strong>the</strong>se young people <strong>of</strong> <strong>the</strong>interest and relevance <strong>of</strong> our subject. We also have to convince course organisers. The steady <strong>de</strong>crease inmedically trained doctors teaching pre-clinical subjects and <strong>the</strong>ir rep<strong>la</strong>cement by scientists interested in animalsfor research and not humans, not to speak <strong>of</strong> <strong>the</strong> enthusiasts who insist that behavioural studies are moreimportant than chemistry, has ma<strong>de</strong> our acceptance less easy. Such persons are all too <strong>of</strong>ten unsympa<strong>the</strong>tic.Here <strong>the</strong> 'proper doctor' who teaches medical history to un<strong>de</strong>rgraduate stu<strong>de</strong>nts is at a telling advantage over <strong>the</strong>non-medical or socially correct historian. When stu<strong>de</strong>nts ask what a disease is really like for <strong>the</strong> sufferer or for<strong>the</strong> doctor trying to cure it, <strong>the</strong>y want someone with clinical experience. This source <strong>of</strong> tension, too, was allu<strong>de</strong>dto in Galveston, and thanks are due to Dr Cynthia Pitcock, who was gal<strong>la</strong>nt enough to do it.Very many thanks are due to Pr<strong>of</strong>essor Ynez O'Neill, whose world presi<strong>de</strong>ncy has been such a distinguished one, andto Dr Burns and his fellow-organisers for all <strong>the</strong>ir hard work for <strong>the</strong> Galveston Conference. There should undoubtedlybe a tie or scarf on issue, for <strong>International</strong> Conferences' organisers. It is a unique club to be a member <strong>of</strong>!L'edition d'hiver <strong>de</strong> notre revue reste dans <strong>la</strong> ligne tracee par le Dr John Cule, son irremp<strong>la</strong>cable editeur <strong>de</strong> <strong>la</strong>premiere heure. 'Vesalius' continue a se <strong>de</strong>velopper, quoique certains facteurs limitent encore <strong>la</strong> gamme <strong>de</strong>sarticles et <strong>de</strong>s auteurs. Le Congres <strong>International</strong> <strong>de</strong> I'an 2000, qui vient d'avoir lieu dans le Nouveau Mon<strong>de</strong>,a ete un pas dans le bonne direction. II faudra perseverer sur <strong>la</strong> meme voie si on veut voir augmenter le nombre<strong>de</strong> lecteurs afin que notre revue <strong>de</strong>vienne une publication pr<strong>of</strong>itant d'un interet vraiment mondial.A Galveston une attention particuliere a ete accor<strong>de</strong>e a ce qu'on pourrait appeler les aspects «plus mo<strong>de</strong>rnes»<strong>de</strong> I'Histoire <strong>de</strong> <strong>la</strong> Me<strong>de</strong>cine. On a bien <strong>de</strong>crit et analyse les difficultes rencontrees en Amerique du Nord et duSud a propos <strong>de</strong> ce qu'on qualifie aujourd'hui <strong>de</strong> 'besoins <strong>de</strong> <strong>la</strong> Sante Publique'. L'introduction <strong>de</strong> <strong>la</strong> politiquedans <strong>la</strong> me<strong>de</strong>cine a ete bien documentee: les pouvoirs croissants <strong>de</strong> certains administratifs pr<strong>of</strong>anes, <strong>de</strong>venus<strong>de</strong>s tyrans, transformant <strong>la</strong> Sante Publique en prenant <strong>de</strong>s mesures pour faire <strong>de</strong> plus en plus d'economies.Quelques me<strong>de</strong>cins doues d'une perspicacite historique nous ont <strong>de</strong>ja mis en gar<strong>de</strong> contre cette tendance.Ceux d'entre nous, qui sont actifs dans <strong>la</strong> formation <strong>de</strong>s etudiants en me<strong>de</strong>cine, savent tres bien qu'il fautconvaincre ces jeunes gens <strong>de</strong> I'interet et <strong>de</strong> <strong>la</strong> pertinence <strong>de</strong> notre matiere. II nous faut aussi en persua<strong>de</strong>r ceuxqui organisent les cours. La diminution constante du nombre <strong>de</strong> me<strong>de</strong>cins-pratiquants dans I'enseignement <strong>de</strong>smatieres pre-cliniques et leur remp<strong>la</strong>cement par <strong>de</strong>s savants qui s'interessent plus a faire <strong>de</strong>s recherches et <strong>de</strong>sexperimentations sur <strong>de</strong>s animaux que sur <strong>de</strong>s etreshumains, sans parler<strong>de</strong>certainespersonnesenthousiastes,qui insistent sur le fait que les etu<strong>de</strong>s behavioristes sont plus importantes que celles <strong>de</strong> chimie, ont contribue anous montrer tres critiques a leur egard: certains d'entre eux ne nous sont parfois pas fort sympathiques.Sous cet aspect, le «veritable me<strong>de</strong>cin», qui enseigne I'Histoire <strong>de</strong> <strong>la</strong> Me<strong>de</strong>cine aux etudiants, jouit d'un grandavantage sur I'historien non-medical ou sur le specialiste en sciences humaines. Quand les etudiants veulentsavoir ce que represente une ma<strong>la</strong>die pour le ma<strong>la</strong><strong>de</strong> ou pour le me<strong>de</strong>cin qui essaye <strong>de</strong> <strong>la</strong> guerir, il s'atten<strong>de</strong>nta se I'entendre expliquer par quelqu'un possedant une experience clinique. On a parle a Galveston <strong>de</strong> cettesource <strong>de</strong> tension et nous <strong>de</strong>vons remercier Madame le Pr<strong>of</strong>esseur Cynthia Pitcock d'avoir eu le courage <strong>de</strong><strong>la</strong>ncer le <strong>de</strong>bat.Nos remerciements vont egalement a Madame le Pr<strong>of</strong>esseur Ynez Viole O'Neill, qui vient d'acheverbril<strong>la</strong>mment son mandat <strong>de</strong> presi<strong>de</strong>nte intemationale, ainsi qu'a Monsieur le Pr<strong>of</strong>esseur Chester Burns et ases col<strong>la</strong>borateurs, qui ont accompli un excellent travail pour organiser le Congres <strong>de</strong> Galveston.
