SPECULUM 31 Medical Journal accused the honorary medical staff of neglecting students, only one of its members, James Rudall, denied the charge. If only because the hospital was paid for admitting students, the committee of management was worried by 1870 at the reluctance of its medical staff to become teachers. All members of the medical staff were therefore presented with a memorandum requesting them to deliver clinical lectures. The reply of the honorary physicians and surgeons was cool. They were prepared to give their time and skill without charge for the care of the sick poor. It was a different thing, however, for them to become formal teachers of students whom they had never sought. William Gillbee spoke for his colleagues when he remarked that medical gentlemen could not be expected to give lectures which would take considerable time and trouble to prepare, and for which they would receive no money and little thanks. Some honorary medical officers gave academic lectures to students in the university medical school itself; but their case was different. Edward Barker, Richard Eades and James Robertson had applied for their lectureships, were paid for them, and were recognised as members of the university staff. The formal request for clinical lectures, on the other hand, was issued from the hospital and not from the university. If the university made what seemed to doctors at the hospital a proper request, and offered proper terms, they would consider it. Meanwhile they were willing merely to give informal teaching to any student who requested it. They refused to be lecturers. The committee of management could not circumvent the opposition of the medical staff by appointing doctors who were willing to be diligent clinical lecturers, since the power to choose medical officers was vested solely in subscribers to the hospital. Lecturers in the university medical school were unable to give clinical examples from the wards unless they happened also to be honorary medical officers to the hospital, for only then were they permitted to enter it. Teaching at the hospital during the 1870's dissatisfied the committee of management, the university council, the faculty of medicine and, most of all, the medical students. "No instruction is given," said one of them in 1874, "the student merely trots around and picks up the crumbs that fall from the table." This was the system described by its authors as informal teaching. Students complained at the small return for their clinical fees, which were higher than in many European hospitals. If they went to Europe after graduating they found themselves handicapped by their lack of bedside experience and by the consequently poor standing of Melbourne degrees. It was not surprising that many Victorians preferred to study medicine somewhere in the United Kingdom where a shorter course would give them a more reputable degree. Nor was it remarkable that students in Melbourne rarely bothered to attend the hospital when their supposed teachers were so truant. The university, as if to condemn as useless what little instruction was given, did not trouble to find out whether medical students attended the hospital, and gave them no examination in clinical methods. Delegates from the university and the hospital met in 1875 to discuss a situation which both parties agreed was intolerable. They hoped to design a plan for affiliation of the two institutions, but their conference had only minor results. A European practice was adopted by which the best students became clinical clerks to physicians and clinical dressers to surgeons. Resident medical officers were to be appointed for a year only, and preference was to be given to the best local graduates of that year. In this way some young doctors would acquire, after graduation, the practical knowledge which they had been denied during their course. On paper, this conference achieved more. A rule was adopted at the hospital which required members of the medical staff to give adequate bedside instruction and at least once a month to deliver a clinical lecture. Like the request that preceded it, the order was ignored. Significantly, the most diligent instruction at the hospital by 1880 was given in the out-patients' department; it was a Listerian cell, where Girdlestone, Webb and Le Fevre enthusiastically taught the techniques of antisepsis to any student on whom they could lay their carbolised hands.
