The difficulties in obtaining a good
chemical contraceptive are that in vitro
conditions are far removed . from clinical
conditions, and a substance which is an excellent
spermicidal on a microscope slide is
often quite useless in the vagina, and substances
which are not impressive in the
laboratory may actually be of great value
as contraceptives.
When a large group of married couples
are organised for actual testing of a contraceptive,
it is difficult to ascertain whether
the instructions for its usage are adhered
to, or indeed, used at all.
Finally, the greatest difficulty with the
contraceptive devices discussed (with
perhaps the exception of the Grafenberg
ring) , is the psychological factor. Having
to take "time off" to adjust a contraceptive
device very often leads to frustration and
inhibition of orgasm. With the diaphragm
it is better if the wife makes a habit of
inserting it in position EVERY night
before going to bed, as a part of the normal
toilet whether coitus is expected or not. By
this method its insertion is a normal procedure,
just as cleaning the teeth, and so the
psychological factor is greatly lessened.
