SPECULUM 6t SURGERY Professor Ewing DIAGNOSTIC QM 1. A young man of 30 received a head injury in a motor car accident. He was believed to have a fracture of the base of the skull but regained consciousness after a short interval. A few days later, however, he began to complain of an unremitting swishing noise in the head, which made sleep impossible. At the same time his right eye became prominent and he developed diplopia. What was the pathological basis for his symptoms? 2. She was a stout woman and her hysterectomy was a difficult and blood one. A blood transfusion was given in the theatre in somewhat of a hurry in the left arm. When she recovered from the anaesthetic she found that she could neither bend the elbow nor dorsiflex the wrist on that side. What had happened? 3. The veins at the bend of the elbow seemed obvious enough but the patient complained unduly when the needle was introduced and inordinately as the pentothal was injected. He did, however, fall asleep. Might there be some later troubles? 4. The goitre was a big one and the dissection correspondingly difficult. After the operation the airway did not appear too good and later there was manifest cyanosis and increasing respiratory distress. What possibilities must be borne in mind? 5. A young man of 30 sustained a simple fracture of the shaft of the femur on each side. Following resuscitation he made very good progress until the third day, when he lapsed into unconsciousness and later developed convulsions. A petechial eruption was noticed on the skin over the front of the chest. What was the diagnosis? 6. On the third day after an operation for the repair of a hernia a robust old gentleman of 60 developed a high fever and severe pain in the foot. What have we here? 7. Her husband, who was actively engaged in establishing a milk bar, died sud- denly from a coronary infraction about a month before. She was very much upset and could not sleep until her doctor gave her some tablets. She was admitted to hospital ten days later with anorexia and jaundice which the biochemical tests showed to be obstructive in type. Would you hazard a diagnosis? 8. We first came to know him when we operated for what was believed to be acute appendicitis. In the immediate post-operative period the diagnosis was amended to enteric fever and we did not see him again until two months later when he reported back complaining of pain and acute tenderness over the body of the sacrum. Is this a rare bird? 9. A truck driver, aged 50, came complaining of a painful swelling of his index finger. It seemed to be a bony swelling, originating in the proximal phalanx and an X-ray showed destruction of bone. The finger was otherwise normal save for nicotine staining. He was apparently in good health although he admitted that he had been unable to shake off a cough since an attack of influenza a few weeks previously. Guess what? 10. His cough never left him at peace and he was bringing up mouthfuls of bitter greenish material, with things in it that looked for all the world like grape-skins. He was 55 and had been a radiographer in a metropolitan hospital for over 30 years. Sixpence on it? 11. She was a front seat passenger in a Holden which ran off the road and hit a tree. Her face came into violent contact with the windscreen and when she consulted her doctor next day the eye was closed and her face greatly swollen. When we first saw her a week later she was complaining of numbness of the right upper lip and the diagnosis was obvious enough. What was it? 12. She was a very old lady, stout and very deaf.
