In addition, the Core Planning Team worked on a purpose/mission statement for the<br />

local public health system. Three versions were drafted, but this effort was not<br />

finalized. <strong>The</strong> following is an amalgamation <strong>of</strong> the purpose/mission statements<br />

presented to date:<br />

To promote good health <strong>and</strong> quality <strong>of</strong> life for all Bexar <strong>County</strong> residents by:<br />

1) Preventing <strong>and</strong> controlling disease, injury, <strong>and</strong> disability.<br />

2) Encouraging healthy behaviors <strong>and</strong> lifestyles.<br />

3) Protecting the environment.<br />

4) Ensuring accessible, affordable, <strong>and</strong> effective health care through the efficient<br />

utilization <strong>of</strong> available resources.<br />

Phase 3: <strong>The</strong> Four <strong>MAPP</strong> Assessments<br />

<strong>The</strong> <strong>MAPP</strong> process requires four different assessments in order to obtain the<br />

necessary information for a community health improvement plan. A description <strong>of</strong><br />

each assessment, along with the efforts made to complete them is outlined below.<br />

Community Health Status Assessment (CHSA):<br />

<strong>The</strong> CHSA answers the question, “How healthy are our residents?” <strong>The</strong> results <strong>of</strong> the<br />

CHSA provide the <strong>MAPP</strong> Core Planning Team with important statistical information<br />

regarding the key health indicators <strong>of</strong> the community. Health issues are identified<br />

through the compilation <strong>of</strong> local health data, as well as the assessment <strong>of</strong> changes over<br />

time, differences among population subgroups, or comparisons to peer, state, or<br />

national data.<br />

<strong>The</strong> CHSA was conducted in two parts. <strong>The</strong> first part was performed by the Bexar<br />

<strong>County</strong> Community Health Collaborative (BCCHC) as noted earlier, <strong>and</strong> focused on<br />

behavioral health data. BCCHC contracted with a team <strong>of</strong> public health <strong>and</strong> academic<br />

pr<strong>of</strong>essionals from the University <strong>of</strong> Texas School <strong>of</strong> Public Health, University <strong>of</strong> Texas<br />

Health Science Center, <strong>and</strong> Our Lady <strong>of</strong> the Lake University to perform the<br />

assessment. To augment this data the San Antonio Metropolitan Health District<br />

contributed the second part <strong>of</strong> the CHSA, the annual health pr<strong>of</strong>iles report. This<br />

report compiles valuable information about the overall population through careful<br />

analysis <strong>of</strong> birth <strong>and</strong> death records, communicable disease reports, school statistics <strong>and</strong><br />

a host <strong>of</strong> other sources. <strong>The</strong> combined CSHA effort was started in December 2001<br />

<strong>and</strong> finalized in September 2002.<br />

On October 3, 2002, the CHSA was presented to the San Antonio <strong>City</strong> Council <strong>and</strong><br />

distributed to key community leaders <strong>and</strong> public health partners. In addition, a press<br />

conference was held on that same day to provide the information to the community. It<br />

is now available on the SAMHD <strong>and</strong> BCCHC Web sites <strong>and</strong> can be accessed online at<br />
