• Instruct patient to look at Cornea Photographer’s right earlobe.<br />
• Place the right edge <strong>of</strong> the parallelepiped (Pp) in the center <strong>of</strong> the pupil.<br />
• <strong>Center</strong> the eye in the field <strong>of</strong> view vertically<br />
• Focus carefully on the tear film.<br />
• Instruct the patient to open wide and take 2 photographs.<br />
• Notice where the left edge <strong>of</strong> the first parallelepiped is, and place the right<br />
edge <strong>of</strong> the second parallelepiped at this point (Pp2 is 1 mm left <strong>of</strong> the Pp1).<br />
• Tell the patient to close his or her eyes. Expose one frame.<br />
• Take 2 photographs. NOTE: Sometimes on this photo, the corneal reflex<br />
(Purkinje image 1) is on top <strong>of</strong> the second parallelepiped; if this occurs, have<br />
the patient fixate slightly to the left (nasal) so that this tear reflection moves<br />
just to the left <strong>of</strong> the location <strong>of</strong> Pp2.<br />
• Move the light tower to 45 degrees nasal to the microscope.<br />
• Place the left edge <strong>of</strong> Pp3 in the center <strong>of</strong> the pupil and position the eye in<br />
the center <strong>of</strong> the field <strong>of</strong> view vertically<br />
• Focus on the tear film and instruct the patient to open his or her eyes wide.<br />
• Take 2 photographs <strong>of</strong> Pp3.<br />
• Tell the patient to close his or her eyes. Expose one frame.<br />
• Move the slit lamp 1 mm nasal and take 2 photos <strong>of</strong> Pp4.<br />
• Photograph the bridge <strong>of</strong> the nose.<br />
• Proceed to the left eye and follow the above procedure with the patient<br />
fixating the Cornea Photographer’s left earlobe.<br />
4) OBLIQUE PHOTOGRAPHS (Obl T and Obl N).<br />
• After full dilation <strong>of</strong> pupil, open the slit beam to full horizontal and vertical<br />
dimensions. (Note: topical corneal anesthetic can be instilled before the<br />
mydriatic drops, but it is not required.)<br />
• Put the neutral density filter in place and move the background illuminator<br />
intensity to <strong>of</strong>f.<br />
• Change the flash intensity to 5 and the magnification to 25X.<br />
• Place the light tower 50 degrees temporal.<br />
• Position the patient for comfort.<br />
• Focus on the central tear film and ask the patient to fixate the Cornea<br />
Photographer’s right earlobe and to open his or her eyes wide.<br />
• Take 2 photographs.<br />
• Move the light tower to 30 degrees nasal.<br />
• Take 2 photographs.<br />
• Photograph the bridge <strong>of</strong> the nose.<br />
• Move to the left eye.<br />
• Repeat the steps above for left eye with patient fixating the Cornea<br />
Photographer’s left earlobe.
