meeting.<br />
12.6 Certification for Slit Lamp Biomicroscopy<br />
Each Clinic must have two CLEK Study-certified people for slit lamp<br />
biomicroscopic examination. Certification requirements for the slit lamp<br />
examination protocol are:<br />
• To attend a CLEK Study training session concerning the design <strong>of</strong> the CLEK<br />
Study and the methods for corneal assessment and documentation;<br />
• To read the CLEK Operations Manual, especially Chapter 7, “Corneal Slit Lamp<br />
Examination” (as well as Chapters 2 and 3), and to learn the documentation <strong>of</strong><br />
corneal scarring and staining on the CLEK Examination Form;<br />
• To review CLEK Study data collection forms, especially the CLEK Examination<br />
Form;<br />
• To complete a written examination on slit lamp biomicroscopy; and<br />
• To complete an in-person or telephone training session.<br />
After satisfactory completion <strong>of</strong> all requirements, verification <strong>of</strong> certification<br />
is issued and forwarded by the Chairman’s Office to the <strong>Coordinating</strong> <strong>Center</strong> and<br />
the Executive Committee. To keep certification current, the certified task must be<br />
performed satisfactorily on at least 10 Study patients in a calendar year.<br />
Probationary certification is granted in the event <strong>of</strong> unsatisfactory<br />
performance. Full certification must be restored within a reasonable period <strong>of</strong> time,<br />
or termination from the Study should be considered.<br />
Annual recertification may be granted at an annual First Investigators Group<br />
meeting.<br />
12.7 Certification for Fluorescein Protocol<br />
Each CLEK Participating Clinic must have at least two people certified for the<br />
fluorescein protocol, including FDACL, contact lens fit assessment, and fluorescein<br />
