• To complete a written examination on keratometry; and<br />

• To complete an in-person or telephone keratometry training session.<br />

After satisfactory completion <strong>of</strong> all requirements, verification <strong>of</strong> certification<br />

is issued and forwarded by the Chairman’s Office to the <strong>Coordinating</strong> <strong>Center</strong> and<br />

the Executive Committee to be used on all CLEK Study forms completed. To keep<br />

certification current, this certified task must be performed on at least 10 Study<br />

patients in a calendar year.<br />

Probationary certification is granted in the event <strong>of</strong> unsatisfactory<br />

performance. Full certification must be restored within a reasonable period <strong>of</strong> time,<br />

or termination from the Study should be considered.<br />

Annual recertification may be granted at an annual Full Investigators Group<br />

meeting.<br />

12.5 Certification for Videokeratography<br />

Each CLEK Participating Clinic must have at least two people certified for<br />

corneal topography measurement. Certification requirements for videokeratography<br />

are:<br />

• To attend a CLEK Study training session concerning the design <strong>of</strong> the CLEK<br />

Study and the methods for corneal curvature assessment and documentation;<br />

• To read the CLEK Operations Manual, especially Chapter 8, “Corneal Curvature<br />

and Topography” (as well as Chapters 2 and 3), and to learn video-keratography<br />

measurement and disk storage techniques;<br />

• To complete a written examination on videokeratography; and<br />

• To complete an in-person or telephone topography training session.<br />

After satisfactory completion <strong>of</strong> all requirements, verification <strong>of</strong> certification<br />

is issued and forwarded by the Chairman’s Office to the <strong>Coordinating</strong> <strong>Center</strong> and<br />

the Executive Committee. To keep certification current, the certified task must be<br />

performed satisfactorily on at least 10 Study patients in a calendar year.<br />
