completion <strong>of</strong> all requirements at the CLEK Photography Reading <strong>Center</strong>, a<br />
certification number is issued by the Project Manager to the CLEK Photograph<br />
Readers.<br />
Probationary certification is granted in the event <strong>of</strong> unsatisfactory<br />
performance. Full certification must be restored within a reasonable period <strong>of</strong><br />
time, or termination from the Study should be considered.<br />
Annual recertification is granted after examination by the CLEK<br />
Photography Reading <strong>Center</strong> Director.<br />
14.5.2 Certification <strong>of</strong> CLEK Photography Reading <strong>Center</strong> Coordinator<br />
Certification requirements for the CLEK Photography Reading <strong>Center</strong><br />
Coordinator are as follows:<br />
(1) Attend training sessions conducted by the Director <strong>of</strong> the CPRC<br />
concerning the design <strong>of</strong> the CLEK Study, the methods for fluorescein<br />
photography, the methods <strong>of</strong> fluorescein photograph reading and<br />
documentation, the methods for corneal photography, and the methods for<br />
cornea photograph reading and documentation;<br />
(2) Study and know in detail the CLEK Operations Manual, especially<br />
Chapters 2, 10, 11, and 14, and the CPRC Operations Manual;<br />
(3) Complete a written examination for the CLEK Photography Reading<br />
<strong>Center</strong> Coordinator as presented by the CPRC Director and the <strong>Coordinating</strong><br />
<strong>Center</strong> Director; and<br />
(4) Demonstrate understanding <strong>of</strong> the CPRC computer system, the flow <strong>of</strong><br />
materials and data between the CPRC and the Clinics, film log recording, slide<br />
labeling, organizing slides for reading, performance <strong>of</strong> data entry and report<br />
generation, sending data to the <strong>Coordinating</strong> <strong>Center</strong>, and slide storage.<br />
All documentation is submitted to the Project Manager at the<br />
<strong>Coordinating</strong> <strong>Center</strong>. The CPRC Director and the <strong>Coordinating</strong> <strong>Center</strong> Director<br />
are responsible for recommending certification. After the written examination,<br />
provisional certification is awarded to the CPRC Coordinator. After satisfactory<br />
completion and demonstration <strong>of</strong> all requirements at the CPRC, a certification<br />
number is issued by the Project Manager to the CPRC Coordinator.
