Quality ReportAnnex B - Statement ofdirectors’ responsibilities inrespect of the Quality ReportThe directors are required under theHealth Act 2009 and the National HealthService (Quality Accounts) Regulations2010 to prepare Quality Accounts foreach financial year. Monitor has issuedguidance to NHS foundation trust boardson the form and content of annualQuality Reports (which incorporate theabove legal requirements) and on thearrangements that foundation trustboards should put in place to supportthe data quality for the preparation of theQuality Report.In preparing the Quality Report, directorsare required to take steps to satisfythemselves that:l the content of the Quality Reportmeets the requirements set out in theNHS Foundation Trust AnnualReporting Manual 2012/13l the content of the Quality Report is notinconsistent with internal and externalsources of information including:l Board minutes and papers for theperiod April 2012 to June 2013l Papers relating to quality reportedto the Board over the period April2012 to June 2013l Feedback from commissionersdated 21/05/2013l Feedback from governors dated21/05/2013l Feedback from Local Healthwatchorganisations dated 22/05/2013l The Trust’s complaints reportpublished under regulation 18 ofthe Local Authority Social Servicesand NHS Complaints Regulations2009, dated 20/05/2013llllThe latest national inpatientsurveyThe latest national staff surveyThe Head of Internal Audits annualopinion over the Trusts controlenvironment dated 24/05/2012CQC quality and risk profilespublished April 2012, June 2012,July 2012, August 2012, October2012, November 2012, December2012, February 2013, March 2013.l the Quality Report presents abalanced picture of the Trust’sperformance over the period covered;l the performance information reportedin the Quality Report is reliable andaccurate;l there are proper internal controls overthe collection and reporting of themeasures of performance included inthe Quality Report, and these controlsare subject to review to confirm thatthey are working effectively inpractice;l the data underpinning the measuresof performance reported in the QualityReport is robust and reliable, conformsto specified data quality standards andprescribed definitions, is subject toappropriate scrutiny and review; andl the Quality Report has been preparedin accordance with Monitor’s annualreporting guidance (whichincorporates the Quality Accountregulations) (published atwww.monitor-nhsft.gov.uk/annualreportingmanual)as well as thestandards to support data quality forthe preparation of the Quality Report(available at www.monitor-nhsft.gov.uk/annualreportingmanual).Annual Report and Accounts 2012/13 101
