James Kwakprovisions, including health insurance subsidies and an expansion<strong>of</strong> Medicaid eligibility, the Affordable Care Act <strong>of</strong> 2010 includedseveral provisions that would reduce growth in Medicare spendingby about $400 billion over ten years. 56 Among others, these includeda reduction in payments to Medicare Advantage plans, changes tothe formulas for calculating reimbursement rates, increasing Part Bpremiums for higher-income beneficiaries, and reducing paymentsto disproportionate share hospitals. 57 Before and since its passage,Republicans have attacked the Affordable Care Act for cutting Medicare,with presidential nominee Mitt Romney recently claiming thatPresident Obama “has taken a series <strong>of</strong> steps that end Medicare aswe know it.” 58147In 2011, the House Budget Committee, led by Representative PaulRyan, proposed converting Medicare into a program where beneficiarieswould receive vouchers they could use to buy coverage fromprivate insurers—but the vouchers would grow in value considerablymore slowly than likely health care inflation, shifting both costsand risks onto beneficiaries. In 2012, the same committee issued aslightly modified proposal in which vouchers would increase in valuewith growth in per capita GDP plus 0.5 percent points, again moreslowly than expected health care inflation. 59 After Romney endorsedthe Ryan plan, President Obama responded by saying that the planwould “end Medicare as we know it.” 60This is not the place to discuss which Medicare proposal is betterpolicy. The point here is that each party positively relishes the opportunityto attack the other for cutting Medicare.
