Appendix OneCensus QuestionnaireThe National University <strong>of</strong> Ireland, Galway has been asked by the National Councilon Age<strong>in</strong>g and <strong>Older</strong> <strong>People</strong> to conduct a survey <strong>of</strong> public, private and voluntarylong-stay facilities <strong>for</strong> older people <strong>in</strong> Ireland. This national project seeks to explorequality <strong>of</strong> care and quality <strong>of</strong> life from the perspectives <strong>of</strong> residents and staff <strong>in</strong> alllong-stay facilities <strong>in</strong> Ireland. This <strong>in</strong><strong>for</strong>mation will be used to guide policy <strong>for</strong> futuredevelopment <strong>in</strong> this sector and your views are critical to this process.244We are ask<strong>in</strong>g you to fill <strong>in</strong> this questionnaire at any stage dur<strong>in</strong>g the weekbeg<strong>in</strong>n<strong>in</strong>g 4 April and to return it <strong>in</strong> the stamp-addressed envelope provided by 11April 2005. Your response matters <strong>in</strong> gett<strong>in</strong>g a detailed picture <strong>of</strong> quality <strong>of</strong> life andquality <strong>of</strong> care <strong>for</strong> older people. The <strong>in</strong><strong>for</strong>mation you provide will be treated <strong>in</strong> strictconfidence and it will not be possible to identify your specific response or yourfacility <strong>in</strong> the f<strong>in</strong>al report.The study has the support <strong>of</strong> all public, private and voluntary agencies <strong>in</strong>volved <strong>in</strong>long-stay care <strong>of</strong> older people <strong>in</strong> the country. The Consultative Committee <strong>for</strong> thestudy <strong>in</strong>cludes representation from the follow<strong>in</strong>g group<strong>in</strong>gs: private nurs<strong>in</strong>g homes,voluntary nurs<strong>in</strong>g homes, the Health Services Executive, the National Council onAge<strong>in</strong>g and <strong>Older</strong> <strong>People</strong>, the Alzheimer Society <strong>of</strong> Ireland and the Department<strong>of</strong> Health and Children.While we recognise that some people may use your facilities <strong>for</strong> respite care,rehabilitation and so on, the focus <strong>of</strong> this study is on people liv<strong>in</strong>g permanently<strong>in</strong> long-stay facilities. There<strong>for</strong>e, we would appreciate if, <strong>for</strong> the questions aboutresidents, you could provide us with <strong>in</strong><strong>for</strong>mation on those long-stay residents onlywhen complet<strong>in</strong>g this questionnaire.Most questions can be answered by tick<strong>in</strong>g one or more boxes or enter<strong>in</strong>g briefdata.
