Traditionally, the quality focus <strong>in</strong> residential care sett<strong>in</strong>gs <strong>in</strong> Ireland has been onquality <strong>of</strong> care rather than quality <strong>of</strong> life. The Health (Nurs<strong>in</strong>g Homes) Act, 1990,provides a regulatory framework that emphasises structural and process dimensions<strong>of</strong> quality <strong>of</strong> care. The ma<strong>in</strong> areas covered by the regulations <strong>in</strong>clude the care andwelfare <strong>of</strong> residents, staff<strong>in</strong>g, accommodation, facilities, hygiene, <strong>in</strong>spectionsand compla<strong>in</strong>ts – all <strong>of</strong> which contribute to quality <strong>of</strong> life, but none <strong>of</strong> which,even <strong>in</strong> comb<strong>in</strong>ation, determ<strong>in</strong>e quality <strong>of</strong> life. Several quality <strong>in</strong>itiatives havebeen developed and implemented <strong>in</strong> recent years <strong>in</strong> an ef<strong>for</strong>t to improve thequality <strong>of</strong> long-stay care <strong>for</strong> older people, but the dom<strong>in</strong>ant concern <strong>in</strong> most <strong>of</strong>these has been on enhanc<strong>in</strong>g standards <strong>of</strong> care rather than quality <strong>of</strong> life. These<strong>in</strong>itiatives <strong>in</strong>clude the establishment <strong>of</strong> the <strong>in</strong>terim Health In<strong>for</strong>mation and <strong>Quality</strong>Authority (iHIQA), the Irish Health Services Accreditation Board (IHSAB), theHealthy Age<strong>in</strong>g Initiative <strong>for</strong> Residential <strong>Care</strong> (HAIRC) (which directly covers quality<strong>of</strong> life dimensions) and various Health Services Executive (HSE) programmes,based ma<strong>in</strong>ly on Essence <strong>of</strong> <strong>Care</strong> frameworks <strong>for</strong> older people <strong>in</strong> long-stay care.36<strong>Long</strong>-stay care units are more than just providers <strong>of</strong> healthcare; they are also thepermanent dwell<strong>in</strong>g place <strong>of</strong> residents – home <strong>for</strong> the people that live there. Theyhave the potential, there<strong>for</strong>e, to <strong>in</strong>fluence and affect many aspects <strong>of</strong> residents’daily lives beyond health and functional ability. It is important, there<strong>for</strong>e, thatpolicy-makers and other <strong>in</strong>terested parties recognise and understand the importantelements that constitute a good quality <strong>of</strong> life <strong>in</strong> long-stay care <strong>in</strong> order to put <strong>in</strong>place mechanisms that will ensure that this objective is achieved.The orig<strong>in</strong> <strong>of</strong> this research report lies <strong>in</strong> the concern <strong>of</strong> the NCAOP about the qualityand effectiveness <strong>of</strong> long-stay care services <strong>in</strong> Ireland and its desire to developquality assurance mechanisms <strong>for</strong> long-stay care that emphasise quality <strong>of</strong> life asmuch as quality <strong>of</strong> care. The purpose <strong>of</strong> the report is to explore quality <strong>of</strong> life issuesas perceived by residents and staff <strong>in</strong> long-stay care sett<strong>in</strong>gs <strong>in</strong> Ireland.The report beg<strong>in</strong>s with an exam<strong>in</strong>ation <strong>of</strong> quality <strong>of</strong> care and quality <strong>of</strong> life issues<strong>for</strong> older people <strong>in</strong> Ireland. Exist<strong>in</strong>g measurement <strong>in</strong>struments and regulatoryframeworks <strong>for</strong> quality <strong>of</strong> care and quality <strong>of</strong> life are described. A cross-countrycomparative review <strong>of</strong> public policy relat<strong>in</strong>g to quality <strong>of</strong> life <strong>in</strong> long-stay care isalso provided. Chapter Two exam<strong>in</strong>es various approaches to understand<strong>in</strong>g quality<strong>of</strong> life be<strong>for</strong>e go<strong>in</strong>g on to explore quality <strong>of</strong> life <strong>for</strong> older people. A number <strong>of</strong> quality<strong>of</strong> life frameworks <strong>for</strong> older people <strong>in</strong> long-stay care are exam<strong>in</strong>ed with a viewto ascerta<strong>in</strong><strong>in</strong>g their usefulness <strong>in</strong> explor<strong>in</strong>g quality <strong>of</strong> life <strong>in</strong> an Irish context <strong>in</strong>Chapter Three.
