There<strong>for</strong>e, it should not be taken<strong>for</strong> granted that dementia sufferers are unable to report on their own quality <strong>of</strong> life.Indeed, encourag<strong>in</strong>g dementia sufferers to activate and use any reta<strong>in</strong>ed abilitiesto communicate can serve to improve the quality <strong>of</strong> life <strong>of</strong> people with dementia(O’Shea and O’Reilly, 1999). Through <strong>in</strong>creased communication, dementia patientscan make their needs and feel<strong>in</strong>gs better understood and have improved<strong>in</strong>teractions with other people and the environment.In cases where dementia patients are <strong>in</strong>capable <strong>of</strong> report<strong>in</strong>g their own quality<strong>of</strong> life, proxy assessments may be utilised. Family members are usually the bestproxies but staff members may also be tra<strong>in</strong>ed to be reliable proxies. Proxy rat<strong>in</strong>g,however, poses a difficult undertak<strong>in</strong>g as proxies must attempt to rate the quality<strong>of</strong> life <strong>of</strong> the resident from the resident’s perspective, not from the perspective<strong>of</strong> the rater themselves (Coen, 1999). This means that a degree <strong>of</strong> objectivity andimpartiality is needed. Add<strong>in</strong>gton-Hall and Kalra (2001) found that proxy assessmentsmay be <strong>in</strong>fluenced by the feel<strong>in</strong>gs and experiences <strong>of</strong> car<strong>in</strong>g <strong>for</strong> the patient.As a group, proxies tend to rate a patient’s quality <strong>of</strong> life worse than the patientthemselves would rate it. Kane (2003c) found that proxy reports correlated poorlywith those <strong>of</strong> residents thereby rais<strong>in</strong>g concern about the validity <strong>of</strong> such measures.713.5 <strong>Quality</strong> <strong>of</strong> <strong>Life</strong> Doma<strong>in</strong>s <strong>for</strong> <strong>Older</strong> <strong>People</strong>In recent decades there has been an <strong>in</strong>creas<strong>in</strong>g acknowledgment <strong>of</strong> the importance<strong>of</strong> the measurement and enhancement <strong>of</strong> quality <strong>of</strong> life <strong>of</strong> older people <strong>in</strong> particular.Increases <strong>in</strong> life expectancy <strong>in</strong> the twentieth century have resulted <strong>in</strong> an <strong>in</strong>creasedolder population <strong>in</strong> Western developed countries. Accompany<strong>in</strong>g this has been anemergence <strong>of</strong> a more positive view <strong>of</strong> old age as a period when people are freedfrom <strong>for</strong>mal social responsibilities such as employment and parent<strong>in</strong>g and are freeto enjoy an active and fulfilled retirement. This means that older people will striveto ma<strong>in</strong>ta<strong>in</strong> and enhance the quality <strong>of</strong> their lives <strong>in</strong> order to fully enjoy this stage<strong>of</strong> the life-cycle. In a study carried out by Gabriel and Bowl<strong>in</strong>g (2004) on the quality<strong>of</strong> life <strong>of</strong> older people liv<strong>in</strong>g <strong>in</strong> private households <strong>in</strong> Brita<strong>in</strong>, the ma<strong>in</strong> quality <strong>of</strong> lifethemes that were revealed <strong>in</strong>cluded: hav<strong>in</strong>g good social relationships, help andsupport; liv<strong>in</strong>g <strong>in</strong> a home and neighbourhood that is perceived to give pleasure,feels safe, is neighbourly and has access to local facilities and services <strong>in</strong>clud<strong>in</strong>gtransport; engag<strong>in</strong>g <strong>in</strong> hobbies and leisure activities (solo) as well as ma<strong>in</strong>ta<strong>in</strong><strong>in</strong>g
