3.7 Conclusion78While the literature on quality <strong>of</strong> life is complex, several areas <strong>of</strong> agreement havebeen highlighted <strong>in</strong> this chapter. It is now clear that quality <strong>of</strong> life is a multidimensionalconcept, which cannot be expla<strong>in</strong>ed only <strong>in</strong> medical terms. <strong>Quality</strong> <strong>of</strong> life onlymakes sense if presented <strong>in</strong> a holistic context. <strong>Quality</strong> <strong>of</strong> life also conta<strong>in</strong>s bothsubjective and objective elements, there<strong>for</strong>e there is a need to take account <strong>of</strong>both when measur<strong>in</strong>g the concept. Specifically, the views <strong>of</strong> older people need tobe <strong>in</strong>corporated <strong>in</strong>to any measure <strong>of</strong> quality <strong>of</strong> life. This is so even <strong>for</strong> people withdementia and related cognitive impairments. There is also an emerg<strong>in</strong>g consensuson quality <strong>of</strong> life doma<strong>in</strong>s <strong>for</strong> older people. These <strong>in</strong>clude objective doma<strong>in</strong>s such asthe physical and care environment, physical and mental health, level <strong>of</strong> function<strong>in</strong>gand socioeconomic status; and subjective doma<strong>in</strong>s such as psychological well-be<strong>in</strong>g,autonomy and <strong>in</strong>dependence, purposeful activity, social relationships, spiritualityand identity/sense <strong>of</strong> self. The emergence <strong>of</strong> these doma<strong>in</strong>s confirms the complex,<strong>in</strong>terrelated and multidimensional nature <strong>of</strong> quality <strong>of</strong> life <strong>for</strong> older people.Notwithstand<strong>in</strong>g these po<strong>in</strong>ts <strong>of</strong> agreement, much debate and division stillsurrounds the concept <strong>of</strong> quality <strong>of</strong> life and a def<strong>in</strong>itive framework <strong>for</strong> its def<strong>in</strong>itionand measurement has yet to be agreed.<strong>Improv<strong>in</strong>g</strong> <strong>Quality</strong> <strong>of</strong> <strong>Life</strong> <strong>for</strong> <strong>Older</strong> <strong>People</strong> <strong>in</strong> <strong>Long</strong>-<strong>Stay</strong> <strong>Care</strong> Sett<strong>in</strong>gs <strong>in</strong> Ireland
