It is notonly policy-makers that have to change; management and staff also need to changeand see their roles <strong>in</strong> terms <strong>of</strong> develop<strong>in</strong>g and enhanc<strong>in</strong>g quality <strong>of</strong> life as much asquality <strong>of</strong> care. <strong>Long</strong>-stay care sett<strong>in</strong>gs are not places to die, they are places to liveand live well. The impetus is now on regulators, policy-makers and practitionersto ensure that residents <strong>of</strong> long-stay care are guaranteed the best quality <strong>of</strong> lifepossible <strong>in</strong> these sett<strong>in</strong>gs as well as the best possible quality <strong>of</strong> care.Authors’ Recommendations321. Develop a person-centred ethos <strong>of</strong> care with<strong>in</strong> residential sett<strong>in</strong>gs througheducation and tra<strong>in</strong><strong>in</strong>g <strong>of</strong> management and staff.2. Ensure that registered nurse to resident staff<strong>in</strong>g ratios are high enough to allowthe direct participation <strong>of</strong> nurses <strong>in</strong> the plann<strong>in</strong>g, delivery and supervision <strong>of</strong> care.3. Develop uni<strong>for</strong>m national care standards <strong>for</strong> all types <strong>of</strong> residential care sett<strong>in</strong>gs<strong>in</strong> Ireland.4. Establish an <strong>in</strong>dependent social services <strong>in</strong>spectorate <strong>for</strong> the regulation,<strong>in</strong>spection and monitor<strong>in</strong>g <strong>of</strong> quality <strong>of</strong> care and quality <strong>of</strong> life <strong>in</strong> residential care.5. Ask residents and their families directly about their experiences <strong>of</strong> long-staycare as part <strong>of</strong> the regulatory and <strong>in</strong>spection process.6. Publish regular <strong>in</strong><strong>for</strong>mation on the results <strong>of</strong> <strong>in</strong>spection which <strong>in</strong>cludesreference to, and monitor<strong>in</strong>g <strong>of</strong>, quality <strong>of</strong> life <strong>in</strong> the follow<strong>in</strong>g doma<strong>in</strong>s: careenvironment and ethos <strong>of</strong> care, sense <strong>of</strong> self and identity, connectedness, andmean<strong>in</strong>gful activities and therapies.7. Develop appropriate consultation structures with<strong>in</strong> long-stay sett<strong>in</strong>gs to giveolder people a greater voice <strong>in</strong> how services are structured and delivered.8. Review the appropriateness <strong>of</strong> current care environments and the overallmanagement <strong>of</strong> residents with dementia, with particular reference to the care<strong>of</strong> people with challeng<strong>in</strong>g behaviours.9. Replace exist<strong>in</strong>g, <strong>in</strong>appropriately designed public long-stay facilities with newbuild<strong>in</strong>gs designed to provide accommodation that recognises the <strong>in</strong>dividuality <strong>of</strong> allresidents and allows them to choose their desired mix <strong>of</strong> private and public space.10. Develop an appropriate reward structure <strong>for</strong> residential care sett<strong>in</strong>gs that delivergood quality care and good quality <strong>of</strong> life <strong>for</strong> residents and penalties <strong>for</strong> thosethat provide poor quality <strong>of</strong> care and dim<strong>in</strong>ished quality <strong>of</strong> life <strong>for</strong> residents.<strong>Improv<strong>in</strong>g</strong> <strong>Quality</strong> <strong>of</strong> <strong>Life</strong> <strong>for</strong> <strong>Older</strong> <strong>People</strong> <strong>in</strong> <strong>Long</strong>-<strong>Stay</strong> <strong>Care</strong> Sett<strong>in</strong>gs <strong>in</strong> Ireland
