Supporting a client in processing his or her emotions by offering acceptance, reassurance and/or
encouragement during times of elevated stress. Service is billed in 15-minute increments.
CDALF Elder abuse protection family each 15 minutes
Teaching a family member(s) of an elderly patient to identify and/or avoid behaviors that are physically or
emotionally harmful to the patient. Service is billed in 15-minute increments.
CDALG� Suicide risk reduction group each 60 minutes
Treating patients in a group setting to reduce their risk of suicidal thought(s) and/or action(s). Service is
billed in 60-minute increments per patient.
CDAZZ Undefined mental health counseling narrative required
Any elsewhere undefined mental health counseling, narrative required. To help develop more

CDBAP Psychological testing brief assessment each 15 minutes
Testing a client/patient and providing a brief assessment of his or her psychological, cognitive and
emotional functioning. Service is billed in 15-minute increments.
CDBAQ Psychological testing comprehensive assessment each 15 minutes
Testing a client/patient and providing a comprehensive assessment of his or her psychological, cognitive
and emotional functioning. Service is billed in 15-minute increments.
CDBAR Psychological assessment interpretation each 15 minutes
Interpreting a client/patient’s psychological, cognitive and emotional functioning assessment. Service is
billed in 15-minute increments.
CDBAS Neuropsychological testing each 15 minutes
Performing a neuropsychological functioning test(s) on a client/patient to assess his or her intellectual
capacity, cognitive functioning, achievement potential, learning disability and/or neuropsychological
deficit(s). Service is billed in 15-minute increments.
CDBAT Neuropsychological test interpretation each 15 minutes
Interpreting a client/patient’s neuropsychological test(s). Any practitioner using this code must document
appropriate training and/or certification. Service is billed in 15-minute increments.
CDBAU Psychiatric test and record evaluation each 15 minutes
Evaluating a client/patient’s psychiatric test results and/or hospital records. Service is billed in 15-minute
increments.
CDBAV Activity therapy assessment each 15 minutes
Assessing a client’s level of interest, aptitude and/or functional level for an activity or activities in addition
to his or her recreational activity history to determine if activity therapy should be incorporated into the
treatment plan. Service is billed in 15-minute increments.
CDBAW Functional behavior assessment each 15 minutes
Assessing a client’s unacceptable and/or inappropriate behavior(s) to identify the behavior(s) and
determine why he or she is demonstrating a behavior(s). Service is billed in 15-minute increments.
