Option 3: The Indices There are two alphabetic indices at the back of this manual: 1. Service and Procedure Index, beginning on page 298 2. Remedy, Product and Supply Index, beginning on page 316 Using ABC Codes for Documenting Care and Billing Using Practitioner Identifiers as ABC Code Modifiers Two-character practitioner identifiers (ex. 1A = Chiropractor) can be used to indicate the practitioner type delivering the ABC-coded intervention. See the chart on the inside of the front cover for a complete list of Practitioner Indentifiers. ABC code modifiers assist practitioners and medical billing and coding professionals in linking ABC codes to state scope of practice laws, education, certification and/or training requirements on a per-intervention and per-state basis. These modifiers also help researchers, managers, and business analysts more effectively compile data by practitioner type and compare economic and health outcomes. Please note: The absence of a practitioner identifier/code modifier does not prohibit and/or restrict the use of ABC codes. Any practitioner may use ABC codes, provided that the codes are within his or her legal scope of practice and/or that he or she can document the appropriate training and/or certification. CMS Forms ABC codes and modifiers fit into the procedure code fields of standard forms, including the CMS-1500 and CMS-1450 forms. CMS-1500 is used by practitioners to submit claims to healthcare financing entities (i.e., insurance companies, Medicare, Medicaid, Workers’ Compensation, etc.). CMS-1450 is used for institutional healthcare claims. To obtain electronic CMS forms, visit www.cms.gov/forms/. The sample on page 15 representing the new CMS-1500 form uses ABC codes and modifiers to document acupuncture services provided by a doctor of Oriental Medicine in Maine: Box 21: Use the ICD-9 CM diagnostic codes 780.6 “fever” and 784.0 “headache” Box 24: Code the treatments provided by a doctor of Oriental Medicine in fields 24A through 24J Box 24A: Enter the date of service in the format MM/DD/YY Box 24B: Enter the place of service (11 = Office) Box 24C: Indicate whether this is an emergency service (Y/N) Note: this field no longer indicates the “Type of Service” as before Box 24D: Enter procedure codes, here ABC codes “ACBAE - Simple subsequent 10 minutes in-office Oriental medicine intake” and “CACAB - Cupping each 15 minutes” Box 24E: Enter practitioner identifier “1C - Oriental Medicine or Acupuncture Practitioner” Box 24F: Enter charges in the format $$.¢¢ Box 24G: Enter the number of units or time increments Box 24H: Indicates whether this is a family planning service (Y/N) Box 24J: Enter your unique NPI (national provider identifier) Box 32: Indicate the location of where the service was provided, including name, address and state. Using ABC Codes with Other Code Sets The ABC code set is designed to fill gaps in other code sets and avoids duplicating existing codes. To accurately document a patient encounter, it may be necessary to use ABC codes in conjunction with other code sets. Terminology Maps Pages 265 - 274 of this manual include terminology maps that “crosswalk” ABC codes to codes from two nursing code sets: The Omaha System Intervention Scheme and Clinical Care Classification (CCC). Getting Your National Provider Identifier (NPI) As of May 23rd, 2007, CMS is requiring practitioners to use an NPI to bill for their services. Getting an NPI is free, but not having one can be costly. You can apply for your NPI in one of three ways: 1. For the most efficient application processing and the fastest receipt of NPIs, healthcare practitioners should consider using the web-based NPI application process. They can log onto the National Plan and Provider Enumeration System (NPPES) and apply on line at http://www.getresponse.com/t/4068528/; or 2. Healthcare practitioners can agree to have an Electronic File Interchange (EFI) organization (EFIO) submit application data on their behalf (i.e., through a bulk enumeration process) if an EFIO requests their permission to do so; or, 3. Healthcare practitioners may wish to obtain a copy of the paper NPI Application/Update Form (CMS-10114) and mail the completed, signed application to the NPI Enumerator located in Fargo, ND, whereby staff at the NPI Enumerator will enter the application data into NPPES. The form will be available only upon request through the NPI Enumerator.
