diaphragm. Service is billed in 15-minute increments.
CEFAT Contraceptive capsule implantation each implantation
Implanting a subcutaneous contraceptive capsule into a client/patient’s body part. Service is billed per
implantation.
CEFAU Contraceptive implant removal each removal
Removing an implanted subcutaneous contraceptive capsule from a client/patient’s body part. Service is
billed per removal.
CEFAV Contraceptive capsule reimplantation each reimplantation
Reimplanting a subcutaneous contraceptive capsule into a client/patient’s body part. Service is billed per
reimplantation.
CEFAW Fertility consultation individual each 15 minutes
Consulting with a client/patient regarding his or her fertility. Service is billed in 15-minute increments.
CEFAX Genetic counseling individual each 15 minutes
Providing a client/patient with education and/or information regarding genetics to prevent and/or minimize
risk of his or her passing a genetic disorder(s). Service is billed in 15-minute increments.
CEFAY Infertility counseling individual each 15 minutes
Providing a client/patient with education and/or information regarding infertility to discuss available
treatment(s) for his or her infertility. Service is billed in 15-minute increments.
CEFAZ Conception counseling individual each 15 minutes
Providing a client/patient with education and/or information regarding conception to promote healthy
behaviors to improve and/or maintain his or her reproductive health. Service is billed in 15-minute
increments.
CEFBA Infertility treatment management each 15 minutes
Managing and/or caring for a client/patient during his or her infertility treatment(s). Service is billed in 15minute
increments.
CEFBB Genetic risk factor identification and/or assessment individual each 15 minutes
Identifying and/or assessing a client/patient’s genetic risk factors. Service is billed in 15-minute
increments.
CEFBC Genetic risk factor identification and/or assessment couple each 15 minutes
Identifying and/or assessing a couple’s genetic risk factors. Service is billed in 15-minute increments.
CEFBD Unplanned pregnancy assistance each 15 minutes
Assisting a client/patient with an unplanned pregnancy in receiving counseling and/or proper care. Service
is billed in 15-minute increments.
CEFBE Fertility consultation couple each 15 minutes
Consulting with a couple regarding his, her and/or their fertility. Service is billed in 15-minute increments.
CEFBF Cervical cap fitting and education each 15 minutes
Fitting a client/patient with a cervical cap and teaching her the proper techniques to care for and use her
cervical cap. Service is billed in 15-minute increments.
CEFBG Conception counseling couple each 15 minutes
Providing a couple with education and/or information regarding conception to promote healthy behaviors
to improve and/or maintain their reproductive health. Service is billed in 15-minute increments.
