Introduction<br />
Underreport<strong>in</strong>g of tuberculosis <strong>in</strong> England<br />
S<strong>in</strong>ce 1987, a rise <strong>in</strong> notifications of tuberculosis has been observed <strong>in</strong> England. 1 This<br />
<strong>in</strong>crease is believed to be real, reflect<strong>in</strong>g an <strong>in</strong>crease <strong>in</strong> diagnoses of tuberculosis, rather<br />
than an artefact due to improved report<strong>in</strong>g. 2 Nevertheless, between 7% and 27% of cases<br />
of tuberculosis have been estimated not to be notified <strong>in</strong> the United K<strong>in</strong>gdom (UK). 3 In<br />
1999, a revised national rout<strong>in</strong>e surveillance system for tuberculosis, Enhanced Tuberculosis<br />
<strong>Surveillance</strong>, was <strong>in</strong>troduced to improve the completeness of report<strong>in</strong>g as well as<br />
the <strong>in</strong>formation on reported cases. 4 The aim of this study was to estimate the annual<br />
<strong>in</strong>cidence of tuberculosis <strong>in</strong> England and assess the completeness of report<strong>in</strong>g between<br />
1999 and 2002 us<strong>in</strong>g record-l<strong>in</strong>kage and capture-<strong>recapture</strong> methodology.<br />
The accuracy and completeness of surveillance data can be <strong>in</strong>creased through<br />
record-l<strong>in</strong>kage between datasets of cases reported from different sources. 5-8 This is carried<br />
out rout<strong>in</strong>ely for cases reported <strong>in</strong> Enhanced Tuberculosis <strong>Surveillance</strong> by l<strong>in</strong>k<strong>in</strong>g<br />
notifications with reports of Mycobacterium tuberculosis isolates from the reference<br />
laboratories <strong>in</strong> the UK Mycobacterial Network (MycobNet). The number of cases missed<br />
can then be estimated us<strong>in</strong>g the overlap between the two data sources through capture<strong>recapture</strong><br />
analysis. 9 The preferred capture-<strong>recapture</strong> method entails log-l<strong>in</strong>ear modell<strong>in</strong>g of<br />
at least three l<strong>in</strong>ked data sources. 10-13 The completeness of the different data sources can<br />
be assessed by comparison with the case-ascerta<strong>in</strong>ment, i.e. the total number of patients<br />
observed <strong>in</strong> at least one data source, or the estimated total number of cases. <strong>Capture</strong><strong>recapture</strong><br />
analysis has been used to evaluate surveillance systems of various <strong>in</strong>fectious<br />
diseases <strong>in</strong> the UK. 14-16 The same methodology has been applied to tuberculosis<br />
surveillance <strong>in</strong> studies <strong>in</strong> both the UK and elsewhere. 17-20<br />
<strong>Methods</strong><br />
Case def<strong>in</strong>ition and data sources<br />
For the purpose of estimat<strong>in</strong>g the number of unobserved tuberculosis cases we def<strong>in</strong>ed as<br />
eligible for <strong>in</strong>clusion those active tuberculosis cases first reported to one or more of three<br />
data sources <strong>in</strong> the four years, 1 January 1999 to 31 December 2002. The three data<br />
sources were:<br />
1. Tuberculosis<br />

cases notified through Enhanced Tuberculosis <strong>Surveillance</strong><br />
2. Cases with Mycobacterium tuberculosis complex isolates reported to MycobNet<br />
(Laboratory).<br />
3. Cases admitted to National Health Service hospitals with a first or secondary hospital<br />
discharge code of tuberculosis (International Classification of Disease (ICD-10) code<br />
A15-A19) provided from Hospital Episode Statistics (Hospital).<br />
Two other data sources used for cross-validation will be mentioned later. An <strong>in</strong>terval<br />
of more than one year between entries <strong>in</strong> each of the data sources was considered as a<br />
separate episode of disease. To correct for delays <strong>in</strong> case report<strong>in</strong>g and myco-<br />
