Underreport<strong>in</strong>g of tuberculosis <strong>in</strong> the Netherlands<br />
demonstrated that 80% (143/179) of the local NTM isolates could be found <strong>in</strong> the<br />
national NTM register. Second, patients later diagnosed with disease other than<br />
tuberculosis or NTM were identified and excluded through record-l<strong>in</strong>kage with a dataset<br />
of such patients secondary to the Netherlands Tuberculosis Register (NTR), an extensive<br />
system of voluntary report<strong>in</strong>g by tuberculosis physicians. 18 Third, non-culture-confirmed<br />
patients possibly diagnosed by histopathology exam<strong>in</strong>ation were verified through the<br />
Pathological Anatomy Laboratory Computerised Archive (PALGA), the nation-wide<br />
network and registry of histopathology and cytopathology results <strong>in</strong> the Netherlands.<br />
Excerpts of the histopathology reports of l<strong>in</strong>ked patients were reviewed by a pathologist<br />
and cases with <strong>in</strong>consistent results discarded. F<strong>in</strong>ally, the total set of l<strong>in</strong>ked tuberculosis<br />
registers was l<strong>in</strong>ked to the NTR for verification of the rema<strong>in</strong><strong>in</strong>g non-culture-confirmed<br />
tuberculosis patients and collection of additional variables for cases <strong>in</strong> any of the l<strong>in</strong>ked<br />
registers: nationality (Dutch, non-Dutch), location of tuberculosis (pulmonary,<br />
extrapulmonary) and <strong>in</strong>fectiousness (sputum smear-positive, sputum smear-negative).<br />
Although more complete <strong>in</strong> data the NTR was expected to have a complete overlap with<br />
the notification register (both registers are ma<strong>in</strong>ta<strong>in</strong>ed by the same tuberculosis physicians)<br />
and was deliberately used for the purpose of validation of the conventional notification,<br />
laboratory and hospital tuberculosis registers. 3<br />
Case-ascerta<strong>in</strong>ment, capture-<strong>recapture</strong> analysis and observed and estimated<br />
register-specific coverage rates<br />
The total and stratified observed register-specific coverage rates are def<strong>in</strong>ed as the<br />
number of tuberculosis patients <strong>in</strong> each register divided by the total or stratified caseascerta<strong>in</strong>ment,<br />
expressed as percentage.<br />
The total number of unobserved tuberculosis cases was estimated on the basis of<br />
the cross-validated distribution of the observed cases over the Notification, Laboratory<br />
and Hospital registers. The <strong>in</strong>dependence of registers and other assumptions underly<strong>in</strong>g<br />
capture-<strong>recapture</strong> analysis have been described previously. 10 Interdependencies between<br />
the three tuberculosis registers are probable, caus<strong>in</strong>g possible bias <strong>in</strong> two-source capture<strong>recapture</strong><br />
estimates. Three-source log-l<strong>in</strong>ear capture-<strong>recapture</strong> analysis was employed to<br />
take possible <strong>in</strong>terdependencies <strong>in</strong>to account. 12,15 Estimated register-specific coverage<br />
rates are def<strong>in</strong>ed as the number of tuberculosis patients <strong>in</strong> each register divided by the<br />
estimated total number of tuberculosis patients by capture-<strong>recapture</strong> analysis.<br />
Results<br />
Table 6.1 shows the <strong>in</strong>itial number of cases, the number of cases excluded from the study<br />
before and after record-l<strong>in</strong>kage and the f<strong>in</strong>al number of cases <strong>in</strong> the three tuberculosis<br />
registers <strong>in</strong> the Netherlands <strong>in</strong> 1998. The hospital admission of 12 cases <strong>in</strong> 1997 and 8<br />
cases <strong>in</strong> 1999, all notified <strong>in</strong> 1998, was <strong>in</strong>cluded <strong>in</strong> the data.<br />
Among the 295 near-l<strong>in</strong>ks between the Notification and Laboratory registers,<br />
the “trusted third party” confirmed 267 candidate-pairs as true l<strong>in</strong>ks. Among the<br />
confirmed l<strong>in</strong>ks, 133 candidate-pairs had adm<strong>in</strong>istrative discrepancies, predom<strong>in</strong>antly<br />
(63.8%) <strong>in</strong> the postal code.<br />
