Son muchas las mujeres (y hombres) que expresan su preocupación a la hora de considerar el efectoque la enfermedad y su tratamiento puedan tener sobre la concepción, el embarazo y la descendencia, así como también dela influencia que el embarazo y el parto puedan tener en la evolución de su propia enfermedad. Este artículo revisa la evidenciaexistente en relación <strong>al</strong> efecto recíproco del embarazo y la <strong>EII</strong>, así como la seguridad del tratamiento de la propia <strong>EII</strong> en estecontexto.SummaryInflammatory bowel disease (IBD) is a chronic disorder which clinic<strong>al</strong> onset occurs usu<strong>al</strong>ly in young adults. El 25% de los casos de <strong>EII</strong> se diagnostican en edadpediátrica (1) , siendo la edad más frecuente en el momentodel diagnóstico entre los 20 y 40 años (2) . De estospacientes en edad fértil, un 25% concibe por primeravez después del diagnóstico de <strong>EII</strong> (3) . Es por esto que unapreocupación recurrente en este grupo de pacientes esla influencia que la <strong>EII</strong> pueda tener en su fertilidad y ensu descendencia. Los cambios que su propia enfermedadpueda presentar a consecuencia del embarazo y elparto, e incluso los cambios de medicación que el embarazoy la lactancia puedan acarrear, son también unmotivo frecuente de temores y consultas.Genética y herenciaLamentablemente, la historia familiar es el factor de riesgomás importante para desarrollar <strong>EII</strong>. Si uno de los progenitoresse encuentra afectado, el riesgo de su descendenciade presentar <strong>EII</strong> es de 2 a 13 veces mayor que elde la población gener<strong>al</strong> (4) , y si ambos progenitores estánafectados, el riesgo aumenta hasta el 36% (5) . Ante estehecho se podría plantear la posibilidad de re<strong>al</strong>izar consejogenético en pacientes con <strong>EII</strong>. Sin embargo, la <strong>EII</strong> esuna enfermedad poligénica que requiere, además de lacarga genética, la influencia de otros factores ambient<strong>al</strong>es,muchos aún por dilucidar. Esto explica la prácticaimposibilidad de establecer un consejo genético, a excepciónde la información del riesgo potenci<strong>al</strong> que existe.Sin embargo, se han depositado grandes expectativasen la utilidad que el estudio genético podría tenersobre la predicción evolutiva (y por tanto, con connotacionesterapéuticas) en pacientes ya diagnosticados.FertilidadFemeninaLa literatura disponible sugiere que las mujeres con <strong>EII</strong>en fase inactiva tienen la misma posibilidad de llevar unembarazo a término en condiciones óptimas y de tenerun parto sin mayor incidencia de complicaciones que lapoblación gener<strong>al</strong> (6) . Sin embargo, el número de embarazosen pacientes con <strong>EII</strong> es menor (7) , probablementepor decisión voluntaria y debido a múltiples razones quevan desde un nivel educacion<strong>al</strong> mayor, <strong>al</strong> miedo a transmitirla enfermedad, a la dificultad de relacionarse o <strong>al</strong>os problemas de imagen corpor<strong>al</strong>, hasta el consejo médicoinapropiado que considera el embarazo como unevento “peligroso” en el curso de la <strong>EII</strong> (8,9) .Las mujeres con diagnóstico de <strong>EII</strong> antes de su primerembarazo tienen menor número de embarazos si se comparancon mujeres sanas o con pacientes con diagnós-163 • Enfermedad Inflamatoria Intestin<strong>al</strong> <strong>al</strong> día - Vol. 7 - Nº. 2 - 2008
