Bij een kind met klinische tekens <strong>van</strong> shock is het belangrijk<br />
een onderscheid te maken tussen cardiale en niet cardiale<br />
oorzaken <strong>van</strong> shock. Dit kan men door eenvoudig de<br />
levergrootte te palperen. Wanneer de lever vergroot is, is<br />
verdere vulling tegenaangewezen en zal men sneller moeten<br />
overgaan tot het toedienen <strong>van</strong> inotropica. Bij belangrijk<br />
bloedverlies <strong>voor</strong> of tijdens de partus en tekenen <strong>van</strong> anemie<br />
bij de pasgeborene kan het nodig zijn snel ongekruist O<br />
negatieve packed cells toe te dienen.<br />
STABILISATIE NA REANIMATIE<br />
Na intensieve reanimatie moet het kind verder gestabiliseerd<br />
worden en eventueel overgebracht naar aan neonatale zorgen<br />
afdeling of neonatale intensieve zorgen afdeling. Het kind<br />
wordt best bewaakt met een cardiorespiratoire monitor en<br />
zuurstofsaturatiemeter. Andere parameters zoals bloeddruk,<br />
glycemie, bloedgaswaarden worden nu bepaald en eventueel<br />
kan een radiografie <strong>van</strong> de thorax aangewezen zijn. Het is<br />
hier ook belangrijk de ouders zo goed mogelijk in te lichten<br />
over de toestand <strong>van</strong> hun kind en de verdere zorgen die het<br />
kind zal nodig hebben. Ook in moeilijke omstandigheden<br />
moet al het mogelijke gedaan worden om een eerste contact<br />
tussen de ouders en hun pasgeboren baby toe te laten.<br />
