GENERAL INSPECTION PROCEDURES <strong>HANDBOOK</strong> CHAPTER 2<br />

Chapter 2 - GENERAL PROCEDURES<br />

This section covers the general procedures that should be addressed by inspectors during<br />

any inspection or investigation. Depending on the type of inspection or investigation,<br />

procedures listed in other h<strong>and</strong>books may also apply. Inspectors must provide at least<br />

the minimum documentation, as directed in the paragraphs labeled Documentation<br />

Required. The documentation required for each procedure listed in this chapter shall<br />

include a statement in the inspection notes that the procedure was completed. A<br />

statement that the required procedure was completed, or an “NA” or “Not Applicable”<br />

may be used if the procedure does not apply. A short statement such as “No Violations<br />

Observed” or “NVO” shall be included when no hazards or violations are observed.<br />

Inspectors should report time pertinent to each event on their Weekly Activity Data Form<br />

(<strong>MSHA</strong> Form 7000-36) for all inspection-related activity, including: inspection<br />

preparation, travel, on-site inspection <strong>and</strong>/or investigation activities, office-generated<br />

paperwork such as opening or closing out the event or report writing associated with the<br />

event, <strong>and</strong> review of the Uniform <strong>Mine</strong> File or Electronic Uniform <strong>Mine</strong> File<br />

(UMF/EUMF).<br />

A. Weekly Activity Data Form (<strong>MSHA</strong> Form 7000-36). The Weekly Activity Data Form<br />

is used to add or change data about an inspector’s work <strong>and</strong> is initiated by an AR or<br />

ROE personnel. The Weekly Activity Data Form is filled out by h<strong>and</strong> or using the<br />

Inspectors’ Portable Application for Laptops (IPAL). It is subsequently entered into<br />

the <strong>MSHA</strong> St<strong>and</strong>ardized Information System (MSIS) database. Completion of the form<br />

is required for all AR <strong>and</strong> ROE personnel assigned to District <strong>and</strong> Field Offices.<br />

Enforcement personnel, including Technicians performing inspection assistance<br />

duties, will complete all items on <strong>MSHA</strong> Form 7000-36 when applicable for activity<br />

codes beginning with E Activities E01 through E32 <strong>and</strong> must be used in conjunction<br />

with a mine ID number <strong>and</strong> an event number.<br />

<strong>MSHA</strong> Form 7000-36 fields are listed below <strong>and</strong> the numbers on the list correspond to<br />

the numbering schema on the form. If this form is being completed using IPAL,<br />

numbers 1 through 3 will be filled in using your IPAL profile.<br />

1. Item 1. Name – Your last name, first name <strong>and</strong> middle initial.<br />

2. Item 1a. AR Number – Your Authorized Representative (AR) number, if<br />

applicable.<br />

3. Item 1b. ROE Number – Your Right of Entry number, if applicable.<br />

4. Item 2. Org Code – The Organization Code (Org) is your office code.<br />
