GENERAL INSPECTION PROCEDURES <strong>HANDBOOK</strong> CHAPTER 3<br />

The completion of each procedure shall be documented in the ITS or on the inspection<br />

tracking map, as specified in the documentation required portion applicable to the<br />

header. Information contained in the ITS should be maintained as required by the<br />

applicable inspection header. If a required inspection procedure cannot be fully<br />

completed by the end of the inspection, the inspector’s immediate supervisor shall be<br />

advised prior to closing the event. The documentation required for each procedural<br />

header listed in this chapter should include a statement in the inspection notes that the<br />

procedure was completed (example: first day arrival in advance of starting time). An<br />

“NA” or “Not Applicable” may be used if the header does not apply. A short statement<br />

such as “No Violations Observed” or “NVO” should be included when no hazards or<br />

violations are observed.<br />

Where documentation is required in the ITS, the applicable section should be completed<br />

in its entirety. The ITS, a checklist, or equivalent preprinted form may be used as part<br />

of the inspection notes which should have on the front of each page the date, inspector<br />

initials, <strong>and</strong> page number(s). This must be provided <strong>and</strong> attached to a daily cover sheet.<br />

Inspectors should document pertinent general observations made during their<br />

inspections in the narrative portion of the inspection notes. If additional documentation<br />

is required, these requirements appear beneath procedural headers. No other<br />

documentation is required unless a violation is observed.<br />

Note: Documentation requirements for this chapter shall be satisfied by: (1) recording<br />

the required information in the inspection notes <strong>and</strong> on the mine map, or; (2) recording<br />

the required information in the inspection notes <strong>and</strong> in the ITS.<br />

Tracking Map. A tracking map should be used to record the location of MMU(s) or<br />

name of section(s), approved evaluation/measuring points, <strong>and</strong> each air course<br />

inspected. The appropriate field or district office shall provide the inspector(s) with a<br />

map for the subject mine if the mine operator is unwilling to provide a copy. Line<br />

diagrams will no longer be accepted for tracking purposes. Tracking maps received<br />

from the mine operator may not always show each location of measurement points or<br />

evaluation points where test <strong>and</strong> measurements are required to be performed by the<br />

mine ventilation plan. All required measurement points <strong>and</strong> evaluation points should<br />

be marked for identification on the tracking map at the beginning of each E01<br />

inspection <strong>and</strong> as new measuring points or evaluation points become approved. This<br />

will help assure that the required points are not missed during the inspection. Each<br />

approved measurement point or evaluation point should be marked on the tracking<br />

map <strong>and</strong> identified by the name <strong>and</strong> location of each measurement point or evaluation<br />

point, including each MMU number. Should a new measurement point or evaluation<br />

point become approved after the affected air course has been traveled <strong>and</strong> inspected<br />

during the quarter’s inspection, a go-back would not be necessary just to document the<br />

air reading at that measurement point or evaluation point. Upon reviewing the UMF<br />

prior to the mine inspection, measurement points or evaluation points should be<br />
