1) Item 13.a. Air Samples. Enter the total number of bottle samples collected.<br />

2) Item 13.b. Rock Dust Spot. Do not use this item due to rock dust spots no<br />

longer exist.<br />

3) Item 13.c. Rock Dust Surveys. Enter the number of individual rock dust<br />

samples collected. Each bag collected <strong>and</strong> submitted counts as one sample.<br />
