4) Item 13.d. Respirable Dust. Enter the total number of individual respirable<br />

dust samples collected, both valid <strong>and</strong> invalid.<br />

5) Item 13.e. Noise. Enter the number of individual noise samples.<br />

6) Item 13.f. Other. Include any other type of health sample collected<br />

individually. If this item contains a value, show specific types of samples<br />

taken in Item 17.<br />

Item 14. Impoundments/Refuse Piles. This section pertains to waste deposits that<br />

have identification numbers assigned. Should more than 10 waste deposits be<br />

inspected, additional entries can be made in Item 17.<br />

1) Item 14.a. Number. The complete waste deposit (refuse pile or<br />

impoundment) number is 11 digits. The first 4 digits of this number are made<br />

up of the State abbreviation (2 digits) <strong>and</strong> the District (2 digits). Since these<br />

items either do not change, or can be identified from the mine ID number or<br />

the organization code, they will not be entered on the form. Enter into the<br />

available positions the last seven digits of the waste deposit identification<br />

number that includes the site identification number (5 digits) <strong>and</strong> the number<br />

of the waste deposit at that site (2 digits). If the last two digits are less than<br />

nine, include a zero before the last digit (example – 03).<br />

2) Item 14.b. Field Hazard Classification (FHC). Leave this field blank.<br />

3) Item 14.c. Configuration. Leave this field blank.<br />

Item 15. Prime Independent Contractor Codes (Major Construction). Enter the 3-<br />

or 4-position alphanumeric identifier of the prime/leading independent<br />

contractor(s) who are inspected as major construction operations under Item 12.k.<br />

only. Do not list minor subcontractors.<br />

Item 16. Inspection Results. Record the number of citations, orders, safeguards,<br />

<strong>and</strong> other actions taken during the period covered by the inspection report.<br />

Count only the actions that have the subject event number on them. Include<br />

vacated issuances in the total number of citations issued.<br />

Item 17. Remarks. This section should be used to document deficiencies<br />

associated with the Cleanup Plan, SCSR Storage Plan, Ground Control Plan, types<br />

of surface workings inspected, miscellaneous areas of inspection, special samples<br />

collected or to provide any other information deemed appropriate.<br />

Item 18. Signature <strong>and</strong> Card Number of AR(s) Responsible for Activity. Enter AR<br />

number(s) of the person(s) participating in the event during the period covered by<br />

Release 1 (February 2013) 2-17
