inspector’s completed <strong>and</strong> initialed copy of the form.<br />
20. Item 18. Key Entered By <strong>and</strong> Date – <strong>Mine</strong> <strong>Safety</strong> <strong>and</strong> <strong>Health</strong> Assistants should<br />
certify with their initials <strong>and</strong> date that the data on this form was entered in<br />
MSIS.<br />
21. Item 19. Remarks - Fill in additional information as appropriate. Information<br />
for this field can only be entered by h<strong>and</strong> either on a preprinted or IPAL<br />
generated copy of the form.<br />
22. Item 20. Inspector Initials <strong>and</strong> Date - When complete, initial <strong>and</strong> date the form.<br />
B. Uniform <strong>Mine</strong> File Review. Inspectors shall review <strong>and</strong> sign the Uniform <strong>Mine</strong><br />
File (UMF)/Electronic Uniform <strong>Mine</strong> File (EUMF) just prior to conducting an<br />
inspection or investigation. The type of event <strong>and</strong> the area to be inspected will<br />
dictate the extent of the review, in accordance with the Uniform <strong>Mine</strong> File<br />
Procedures H<strong>and</strong>book. The UMF/EUMF review shall be conducted as part of the<br />
inspection/investigation <strong>and</strong> coded as “other” time. Inspectors who are regularly<br />
assigned to a coal mine are not required to conduct additional reviews of the<br />
UMF/EUMF when conducting other types of inspections or investigations during<br />
the same inspection quarter, but are not precluded from doing so. There are<br />
instances when an inspector conducts a "limited inspection" which would not require<br />
the UMF to be reviewed in its entirety. A “limited inspection” is generally specific in<br />
nature <strong>and</strong> short in duration (one or two days on a certain event) (Example:<br />
ventilation specialists with the intent to inspect an area of their expertise such as<br />
bleeder entries). The inspectors <strong>and</strong> specialists must review for a "limited inspection"<br />
at a minimum, the roof control <strong>and</strong> ventilation plans <strong>and</strong> any other plans pertinent to<br />
that inspection. Immediate signature <strong>and</strong> review does not apply to mine emergencies.<br />
Documentation Required: The inspector should document the date of the UMF review on<br />
the General Information Cover Sheet <strong>and</strong> complete the <strong>MSHA</strong> Form 2000-137 (Inspector<br />
Certification Form) in the UMF/EUMF, per instructions in the Uniform <strong>Mine</strong> File<br />
Procedures H<strong>and</strong>book. If additional reviews are conducted, no additional documentation is<br />
required.<br />
C. Arrival at the <strong>Mine</strong>. Inspectors should arrive at the mine in time for pre-inspection<br />
contacts, a preliminary review of examination records, <strong>and</strong> an overview of the mine<br />
map to determine which area of the mine to begin the inspection or investigation.<br />
While performing surface or underground inspection activity related to an open<br />
event, the mine map shall be thoroughly reviewed with special attention given to<br />
mining in close proximity to worked-out areas, oil <strong>and</strong> gas wells, fuel transmission<br />
lines, <strong>and</strong> bodies of water which could present an underground flood hazard, <strong>and</strong><br />
