GENERAL COAL MINE INSPECTION PROCEDURES AND<br />

INSPECTION TRACKING SYSTEM <strong>HANDBOOK</strong> CHAPTER 4<br />

<strong>MSHA</strong> Form 2000-209 <strong>and</strong> 2000-209 Instruction Sheet, must be used whenever a<br />

change or update in the mine status occurs. (Refer to the Union Abbreviation Codes<br />

for further guidance if filling out line 26.) The form is completed when a mine opens<br />

or closes, or whenever it is necessary to enter or delete a mine from the system; to<br />

change the status of a mine; or to provide or change descriptive information about a<br />

mine.<br />

All other items on this form shall be reviewed for completeness <strong>and</strong> accuracy during<br />

each E01 inspection of the mine. However, it is not necessary to fill out the form<br />

completely during these reviews. Any time the information is being updated, it is<br />

necessary to complete the first four items plus the information that is being changed.<br />

<strong>Mine</strong> operators are required by regulation to notify <strong>MSHA</strong> of certain changes to the<br />

operational status of each mine. However, it is the responsibility of <strong>MSHA</strong><br />

personnel to determine the correct operational <strong>and</strong> auxiliary status of each mine <strong>and</strong><br />

ensure that this status is updated. The mine status should be changed as often as<br />

necessary, either on notification from mine operators or from personal observation.<br />

If the operational <strong>and</strong>/or auxiliary status of a mine clearly differs from the status<br />

provided by the mine operator, <strong>MSHA</strong> personnel shall determine the correct status<br />

<strong>and</strong> update <strong>MSHA</strong> Form 2000-209 to reflect the actual mine status. At a minimum,<br />

both the operational status <strong>and</strong> the auxiliary status of each mine must be reviewed<br />

<strong>and</strong>, if necessary, changed by updating <strong>MSHA</strong> Form 2000-209 at the start of every<br />

inspection event (for mines with multipliable air samples to be conducted to calculate<br />

total methane liberation the form should be completed as soon as the analyses are<br />

received to determine such liberation). Total mine employees on line 15 <strong>and</strong><br />

methane liberation on line 22a often change each quarter <strong>and</strong> an updated MIF needs<br />

to reflect any changes. A copy of the completed Form 2000-209 should be<br />

immediately submitted to the immediate supervisor for review so the information<br />

can be entered in the MSIS system by the appropriate <strong>Mine</strong> <strong>Safety</strong> <strong>and</strong> <strong>Health</strong><br />

Assistant. A copy shall be maintained with the inspection report. The information<br />

from Form 2000-209 should be entered in the MSIS system within 3 working days<br />

after it has been received by the <strong>Mine</strong> <strong>Safety</strong> <strong>and</strong> <strong>Health</strong> Assistant.<br />

Z. General <strong>Mine</strong> Status Information (MMU/DA/DWP Data). The status codes on<br />

<strong>MSHA</strong> Form 2000-142 are changed only when there are changes in the status of the<br />

complete mine. If there is a change in the status of a portion of a mine (MMU, DWP,<br />

DA) that does not apply to the complete mine, that change should be reported on the<br />

MMU/DA/DWP Data Form. The sampling requirements for the operator are based<br />

on the status of the individual sampling entities <strong>and</strong> not the mine itself. The general<br />

definition of a producing underground mine, as given in §§ 70.220 <strong>and</strong> 71.220, is one<br />
