INSPECTION TRACKING SYSTEM <strong>HANDBOOK</strong> CHAPTER 5<br />

of examination st<strong>and</strong>ards that should be probed by the inspector.<br />

Sample bags are long enough to permit tying a knot in the open ends. Securely tie<br />

the string of the identifying tag within the formed knot of the sample bag. Each tag<br />

is pre-printed with a unique alphanumeric code that will be used to identify the<br />

sample. Inspectors may include additional h<strong>and</strong>written information on the tag,<br />

provided it does not obscure the pre-printed identifier. In order to process the<br />

sample, the laboratory needs the pre-printed tag attached firmly to the sample.<br />

It is the responsibility of the NADL supervisor to ensure that rock dust analysis<br />

reports <strong>and</strong> accompanying analysis data are promptly made available for use by<br />

the districts. Software tools allow supervisors <strong>and</strong> others to monitor for prompt<br />

issuance of citations/orders for non-compliance <strong>and</strong> to generate oversight reports.<br />

The responsible supervisor must assure that all rock dust analysis reports returned<br />

from the NADL are included within the appropriate inspection report. A citation<br />

or order must be promptly issued for non-compliant rock dust samples.<br />

Data Submittal <strong>and</strong> Mailing of Bagged Samples<br />

Rock dust sampling data must be input into <strong>and</strong> submitted using the Inspectors<br />

Portable Application for Laptops (IPAL). Rock dust samples should be mailed to<br />

the NADL in accordance with postal regulations within 3 working days from when<br />

the samples were collected. Securely seal the shipping boxes to prevent loss of<br />

samples in transit. Include the return address on the shipping label. Place the<br />

return address of the office on the outside of the shipment box from which the<br />

samples are being mailed. Use a regular corrugated pasteboard carton, but fill<br />

voids around the bags with packing to keep the bags from breaking open from<br />

rough h<strong>and</strong>ling. Do not use crumpled manila envelopes, excelsior, paper towels,<br />

shredded paper, packing peanuts or tissues as packing. Dust Sampling Lab Report<br />

(<strong>MSHA</strong> Form 2000-156 – Rock Dust Sample Submission Form) must be prepared<br />

<strong>and</strong> uploaded to the NADL<br />

Server using the IPAL. A copy of <strong>MSHA</strong> Form 2000-156 must be printed <strong>and</strong><br />

shipped with the bagged sample(s). Compliance/noncompliance concerning rock<br />

dust samples will be determined at the NADL <strong>and</strong> the results returned<br />

electronically.<br />

The NADL supervisor will notify the appropriate District Manager should boxed<br />

samples arrive at the lab <strong>and</strong> no accompanying <strong>MSHA</strong> Form 2000-156 data is<br />

available on the file server. Hard copies of <strong>MSHA</strong> Form 2000-156 will not be<br />
