Students should be aware that anyone who observes any violation <strong>of</strong> the pr<strong>of</strong>essional<br />

standards should report the incident to the Student Conduct Committee. Complaints can be<br />

made confidentially. Every effort should be made to maintain the confidentiality <strong>of</strong> all the<br />

members involved in the alleged incident.<br />

APPLICATION OF PROFESSIONALISM POLICY<br />

Alleged violations <strong>of</strong> the Pharmacy School Pr<strong>of</strong>essionalism Policy including academic issues<br />

involving falsification, collusion, plagiarism or cheating will be resolved through the academic<br />

proceedings <strong>of</strong> the School <strong>of</strong> Pharmacy Student Conduct Committee (SCC).<br />

Organization<br />

This committee serves as a hearing board for violations <strong>of</strong> the Pharmacy School Pr<strong>of</strong>essionalism<br />

Policy. The Pharmacy School Pr<strong>of</strong>essionalism Policy includes standards for academic, nonacademic,<br />

and clinical behavior. The Student Conduct Committee will make recommendations<br />

to the Dean <strong>of</strong> the School <strong>of</strong> Pharmacy. The Dean will then notify the student in writing <strong>of</strong><br />

actions concerning alleged violations. A record <strong>of</strong> disciplinary action normally is maintained by<br />

the Office <strong>of</strong> Student Affairs until the student graduates or leaves the School <strong>of</strong> Pharmacy.<br />

Students may examine the contents <strong>of</strong> their file by appointment with the Assistant Dean for<br />

Student Affairs.<br />

Proceedings Involving Violations <strong>of</strong> Student Conduct<br />

Charges <strong>of</strong> misconduct may arise from a student (or group <strong>of</strong> students), pr<strong>of</strong>essor, adjunct<br />

pr<strong>of</strong>essor, or preceptor. Any suspicion <strong>of</strong> a violation <strong>of</strong> the Pharmacy School Pr<strong>of</strong>essionalism<br />

Policy should be reported to any faculty or administrator for further investigation.<br />

Reporting and Investigating Pr<strong>of</strong>essional Misconduct<br />

Any individual may report alleged academic or pr<strong>of</strong>essional misconduct <strong>of</strong> School <strong>of</strong><br />

Pharmacy student(s). This report should be made to either a School <strong>of</strong> Pharmacy<br />

faculty, staff or administrator. The faculty, staff, or administrator notified <strong>of</strong> these<br />

allegation(s) <strong>of</strong> misconduct will then forward the Student Intervention form and any<br />

dated notes to describe the situation to the Student Conduct Committee Chair, or<br />

designee, as soon as possible but not exceeding five (5) business days. Whenever<br />

possible, the documentation <strong>of</strong> the alleged misconduct should include a personal<br />

interview with the accused student(s), witnesses and pr<strong>of</strong>essor involved. The SCC<br />

Chair will forward a copy <strong>of</strong> the Student Intervention form to the Student Affairs <strong>of</strong>fice.<br />

Every effort should be made to maintain confidentiality in these discussions.<br />

An investigation or appropriate action will be pursued by the SCC Chair, or designee,<br />

within five (5) business days.<br />

After an investigation <strong>of</strong> the allegation(s), the following options are available for<br />

resolution <strong>of</strong> the initial complaint:<br />

1. If it is determined that there is insufficient evidence to support the allegation(s), no<br />

disciplinary action will be taken. However, a form stating this finding will be<br />

forwarded to and placed on file in the Office <strong>of</strong> Student Affairs.<br />

2. If the allegations are substantiated, the case will be referred to the Student Conduct<br />

Committee.<br />

a. After collecting all information relevant to the case, the Chair <strong>of</strong> the Student<br />

Conduct Committee will call for a preliminary hearing within five (5) business<br />
