PROGRESSION TO SP-2 YEAR AND SP-3 YEAR<br />

SP-2 YEAR REQUIREMENTS<br />

Prior to starting the SP-2 year, all <strong>of</strong> the following must be completed:<br />

58 credit hours <strong>of</strong> the SP-1pr<strong>of</strong>essional curriculum<br />

o 50 hours <strong>of</strong> the didactic curriculum<br />

o 8 hours <strong>of</strong> IPPEs<br />

Successful passage <strong>of</strong> the Progression Exam<br />

SP-3 YEAR REQUIREMENTS<br />

Prior to starting the SP-3 Rotations, all <strong>of</strong> the following must be completed:<br />

120.5 credit hours <strong>of</strong> the Pr<strong>of</strong>essional Curriculum<br />

o 100.5 hours pr<strong>of</strong>essional curriculum<br />

o Completion <strong>of</strong> all IPPE Hours<br />

o 6 hours <strong>of</strong> approved pharmacy electives<br />

Successful Passage <strong>of</strong> the Progression Exam<br />

Required annual PPD on file with Division <strong>of</strong> Experiential Education<br />

Required annual physical examination, on file with Division <strong>of</strong> Experiential Education<br />

Requisite CPR Training, on file with Division <strong>of</strong> Experiential Education<br />

Application for Graduation, on file with the Registrar’s Office<br />

CONSEQUENCES OF UNSATISFACTORY ACHIEVEMENT OF ACADEMIC AND/OR<br />

PROFESSIONAL STANDARDS<br />

PROBATION<br />

The Associate Dean for Academic Affairs will place a student who exhibits unsatisfactory or<br />

deficient academic performance or inappropriate pr<strong>of</strong>essional conduct on probation. Students<br />

on probation are required to satisfy and comply with the terms and/or conditions <strong>of</strong> their<br />

probation. The Associate Dean may recommend that the Dean suspend students who have<br />

not fulfilled the terms and conditions <strong>of</strong> their probation.<br />

Academic Probation/Withdrawal “Not in Good Academic Standing”<br />

If a student’s performance is unsatisfactory on any academic program requirements, the<br />

student will be placed on academic probation. The Associate Dean for Academic Affairs will<br />

acknowledge the student’s placement on academic probation and will specify the terms <strong>of</strong><br />

probation in a written document. Included in this document will be:<br />

1. A statement outlining the area <strong>of</strong> deficiency,<br />

2. A statement reiterating the School’s Withdrawal Policy, and<br />

3. The terms and conditions for returning to good academic standards.<br />

This document will be delivered either by certified mail or hand-delivered to the student. The<br />

student must sign a copy acknowledging receipt <strong>of</strong> the letter and return it to the Associate<br />

Dean. A copy <strong>of</strong> the letter will be submitted to the Dean.<br />
