last<strong>in</strong>g 6 hours associated with vomit<strong>in</strong>g and photophobia. She has<br />
had several episodes <strong>in</strong> the past.<br />
The classic description of migra<strong>in</strong>e <strong>in</strong>cludes a preced<strong>in</strong>g aura before the<br />
onset of the headache, but this feature does not occur <strong>in</strong> most patients.<br />

A 56-year-old woman compla<strong>in</strong>s of unilateral stabb<strong>in</strong>g pa<strong>in</strong> on the<br />
surface of her scalp and around her eye. The pa<strong>in</strong> is precipitated by<br />
wash<strong>in</strong>g or touch<strong>in</strong>g the specific area.<br />
This woman gives a classic history of trigem<strong>in</strong>al neuralgia affect<strong>in</strong>g the<br />
ophthalmic division of the trigem<strong>in</strong>al nerve. This condition more commonly<br />
presents <strong>in</strong> the mandibular and maxillary branches. It is pr<strong>in</strong>cipally<br />
a compla<strong>in</strong>t <strong>in</strong> older patients.<br />
A 28-year-old woman compla<strong>in</strong>s of headache and double vision<br />
which is worse when ly<strong>in</strong>g down. Exam<strong>in</strong>ation reveals papilloedema<br />
but no focal signs. There are no abnormal f<strong>in</strong>d<strong>in</strong>gs on CT.<br />
Benign <strong>in</strong>tracranial hypertension is a condition of unknown aetiology<br />
which usually affects obese women. Steroids and tetracycl<strong>in</strong>es are<br />
recognized precipitants.<br />
Diagnosis of raised CSF pressure is made by LP. Symptoms often<br />
resolve after LP. Chronic treatment <strong>in</strong>cludes the use of repeated LPs and<br />
diuretics.<br />
A 30-year-old man compla<strong>in</strong>s of a dull headache that is worse when<br />
ly<strong>in</strong>g down or when cough<strong>in</strong>g. He has recently suffered a seizure.<br />
A description of early morn<strong>in</strong>g headache that is exacerbated by ly<strong>in</strong>g<br />
down/cough<strong>in</strong>g/sneez<strong>in</strong>g is a classic description of raised <strong>in</strong>tracranial<br />
pressure. The development of seizures is a s<strong>in</strong>ister f<strong>in</strong>d<strong>in</strong>g and urgent<br />
imag<strong>in</strong>g is warranted.<br />
A 65-year-old woman compla<strong>in</strong>s of a constant ach<strong>in</strong>g pa<strong>in</strong> around<br />
the right eye radiat<strong>in</strong>g to the forehead. There is reduced vision <strong>in</strong> the<br />
eye, which is red and congested with a dilated pupil.<br />
Acute closed angle glaucoma mostly affects the long-sighted elderly<br />
population. It is caused by blockage of dra<strong>in</strong>age of aqueous humour from<br />
the anterior chamber via the canal of Schlemm.<br />
Stimuli that cause pupillary dilatation (e.g. sitt<strong>in</strong>g <strong>in</strong> the dark) <strong>in</strong>creases<br />
the tightness of contact between the iris and the lens and can precipitate<br />
an attack.
