His chest radiograph shows bilateral hilar lymphadenopathy.<br />

An abnormal <strong>in</strong>cidental chest radiograph f<strong>in</strong>d<strong>in</strong>g or respiratory symptoms<br />

are the <strong>in</strong>itial presentations <strong>in</strong> up to 50 per cent of sarcoid patients.<br />

TB, malignancy, silicosis and extr<strong>in</strong>sic allergic alveolitis are other causes<br />

of bilateral hilar lymphadenopathy. There is a higher <strong>in</strong>cidence of<br />

sarcoidosis among African–Caribbeans.<br />
