worse when the patient is feel<strong>in</strong>g particularly stressed. It is important to<br />
rule out any other pathology before this diagnosis is made.<br />

A 40-year-old man compla<strong>in</strong>s of <strong>in</strong>termittent dysphagia associated<br />
with chest pa<strong>in</strong>. Barium swallow reveals a corkscrew oesophagus.<br />
There is abnormal contraction of the oesophagus giv<strong>in</strong>g rise to the<br />
corkscrew appearance on barium swallow. Anti-spasmodics can be<br />
prescribed but are not always helpful.<br />
A 55-year-old man presents cough<strong>in</strong>g when he tries to swallow. On<br />
exam<strong>in</strong>ation he has a flaccid fasciculat<strong>in</strong>g tongue.<br />
There is difficulty <strong>in</strong> coord<strong>in</strong>at<strong>in</strong>g the swallow<strong>in</strong>g movement. There is<br />
lower motor neuron weakness of the muscles supplied by the cranial<br />
nerve which lie <strong>in</strong> the medulla. The flaccid fasciculat<strong>in</strong>g tongue <strong>in</strong>dicates<br />
nerve XII <strong>in</strong>volvement.<br />

A 62-year-old man presents with rectal bleed<strong>in</strong>g and a year’s history of<br />
left iliac fossa pa<strong>in</strong> and change <strong>in</strong> bowel habit. There is no weight loss.<br />
Diverticular disease ma<strong>in</strong>ly occurs <strong>in</strong> the sigmoid colon. A high-fibre diet<br />
helps to prevent the high <strong>in</strong>tralum<strong>in</strong>al pressures that are believed to<br />
cause diverticula.<br />
An 8-month-old baby presents with <strong>in</strong>consolable cry<strong>in</strong>g, colic and<br />
bleed<strong>in</strong>g per rectum. A sausage-shaped abdom<strong>in</strong>al mass is palpable.<br />
Intussusception is a paediatric surgical emergency and refers to the<br />
<strong>in</strong>vag<strong>in</strong>ation of proximal bowel <strong>in</strong>to a distal segment. The most common<br />
pathology <strong>in</strong>volves the ileum pass<strong>in</strong>g <strong>in</strong>to the caecum and colon through<br />
the ileocaecal valve. This condition usually occurs <strong>in</strong> children between the<br />
age of 2 months and 2 years. Presentation is typically with <strong>in</strong>consolable<br />
cry<strong>in</strong>g, colic (child classically draws the legs up) and a sausage-shaped<br />
mass <strong>in</strong> the abdomen. The rectal bleed<strong>in</strong>g is sometimes described as<br />
hav<strong>in</strong>g a redcurrant jelly-like appearance. The condition is usually treated<br />
by rectal air <strong>in</strong>sufflation. If this fails, surgical <strong>in</strong>tervention is <strong>in</strong>dicated.<br />
A 35-year-old man returns from holiday with a 2-week history of<br />
fever, cramp<strong>in</strong>g abdom<strong>in</strong>al pa<strong>in</strong> and bloody diarrhoea.<br />
The acute history and recent travel history make <strong>in</strong>fectious diarrhoea the<br />
most likely cause. Stool sample for microscopy, culture and sensitivity is<br />
the <strong>in</strong>vestigation of choice. If the patient is systemically unwell, antibiotics<br />
(e.g. empirical treatment with ciprofloxac<strong>in</strong>) may be started<br />
before diagnosis is confirmed.
