patches over her body. Hb 14 g/dl, glucose 3.5 mmol/l, serum Na 128 mmol/l,<br />

serum K 5.4 mmol/l, serum Ca 2 2.6 mmol/l.<br />

3 A 15-year-old boy presents acutely confused with hyperventilation, vomit<strong>in</strong>g<br />

and abdom<strong>in</strong>al pa<strong>in</strong>. Hb 14 g/dl, white cell count 13.0 10 9 /l, glucose 19 mmol/l,<br />

platelets 160 10 9 /l.<br />

4 A 73-year-old woman with a history of metastatic breast cancer presents with<br />

vomit<strong>in</strong>g and abdom<strong>in</strong>al pa<strong>in</strong>. She is found to be acutely confused. On exam<strong>in</strong>ation<br />

positive f<strong>in</strong>d<strong>in</strong>gs <strong>in</strong>clude dry mucous membranes. Hb 10 g/dl, white cell<br />

count 7 10 9 /l, glucose 4.5 mmol/l, platelets 350 10 9 /l, corrected calcium<br />

4.2 mmol/l, serum K 3.5 mmol/l.<br />

5 A 26-year-old woman presents with fever, confusion and agitation. Exam<strong>in</strong>ation<br />

reveals an irregular pulse of rate about 240/m<strong>in</strong> and the presence of a goitre.<br />
