This is a herpes zoster <strong>in</strong>fection (sh<strong>in</strong>gles). Oral aciclovir is the treatment<br />
of choice.<br />
A 41-year-old HIV-positive man presents with decreased visual acuity.<br />
Fundoscopy shows a ‘mozarrella and tomato pizza’ appearance.<br />
Cytomegalovirus (CMV) ret<strong>in</strong>itis usually occurs only when the CD4 count<br />
has dropped to below 100. The diagnosis is made cl<strong>in</strong>ically. Fundoscopy<br />
reveals a characteristic ‘pizza’ appearance caused by the appearance of<br />
haemorrhages and exudates that follow the ret<strong>in</strong>al vessels.<br />
CMV ret<strong>in</strong>itis is a medical emergency as a result of the risk of bl<strong>in</strong>dness<br />
and should be treated with <strong>in</strong>travenous ganciclovir. Foscarnet can also<br />
be used to treat CMV ret<strong>in</strong>itis.<br />
An <strong>in</strong>fant is suffer<strong>in</strong>g from severe respiratory syncytial virus (RSV)<br />
bronchiolitis.<br />
Inhaled ribavir<strong>in</strong> is <strong>in</strong>dicated, although there is not yet clear evidence<br />
that it provides significant cl<strong>in</strong>ical benefit. A monoclonal antibody is<br />
now available that can be used to prevent RSV <strong>in</strong>fection <strong>in</strong> <strong>in</strong>fants at<br />
high risk (palivizumab).<br />
A 40-year-old man with chronic hepatitis C <strong>in</strong>fection.<br />
Interferon- is given by subcutaneous <strong>in</strong>jection. Comb<strong>in</strong>ation therapy<br />
with ribavir<strong>in</strong> is more effective and should be cont<strong>in</strong>ued for 6 months.<br />

AZT, didanos<strong>in</strong>e and <strong>in</strong>d<strong>in</strong>avir provide a comb<strong>in</strong>ation of two nucleoside<br />
sanalogue reverse transcriptase <strong>in</strong>hibitors (AZT, didanos<strong>in</strong>e) with one protease<br />
<strong>in</strong>hibitor (<strong>in</strong>d<strong>in</strong>avir), which is commonly referred to as highly active<br />
anti-retroviral therapy (HAART). Non-nucleoside analogue reverse transcriptase<br />
<strong>in</strong>hibitors, e.g. nevirap<strong>in</strong>e, can also be used as part of HAART.<br />

A 25 year old is rushed to A&E present<strong>in</strong>g with vomit<strong>in</strong>g,<br />
hyperventilation, t<strong>in</strong>nitus and sweat<strong>in</strong>g. Blood gases show a<br />
respiratory alkalosis.<br />
Patients <strong>in</strong>itially present with a respiratory alkalosis caused by<br />
hyperventilation (stimulation of the respiratory centre) but a metabolic<br />
acidosis may develop later.<br />
Multiple doses of oral activated charcoal reduces the absorption of<br />
aspir<strong>in</strong> and is more effective the earlier it is given.<br />
Management of overdose can be classified accord<strong>in</strong>g to<br />
mild/moderate/severe overdose.
