A 26-year-old woman presents acutely unwell with shortness of<br />

breath. Her ECG shows s<strong>in</strong>us tachycardia, deep S waves <strong>in</strong> I, <strong>in</strong>verted<br />

T waves <strong>in</strong> III and Q waves <strong>in</strong> III.<br />

This is the S I, Q III, T III pattern frequently quoted <strong>in</strong> books but it is<br />

actually rare <strong>in</strong> practice. Right axis deviation may be present but the<br />

ECG is often normal <strong>in</strong> small/medium pulmonary emboli.<br />

Dom<strong>in</strong>ant R <strong>in</strong> V1, <strong>in</strong>verted T waves <strong>in</strong> V1–V3, deep wide<br />

S waves <strong>in</strong> V6.<br />

The ‘MARROW’ pattern, e.g. ‘RSR’ pattern <strong>in</strong> V1 (M) with deep wide S<br />

wave <strong>in</strong> V6 (W). Causes <strong>in</strong>clude atrial septal defect (ASD) and pulmonary<br />

embolus.<br />

Prolonged P–R <strong>in</strong>terval, depressed ST, flattened T waves, prom<strong>in</strong>ent<br />

U waves.<br />

Look out for loop/thiazide diuretics as a cause of hypokalaemia.<br />

S<strong>in</strong>us rhythm, bifid ‘p’ waves best seen <strong>in</strong> II, V3 and V4.<br />

Known as P mitrale, this bifid P wave suggests left atrial hypertrophy. A<br />

peaked P wave is called P pulmonale and suggests right atrial hypertrophy.<br />

A 65-year-old man presents with chest pa<strong>in</strong> radiat<strong>in</strong>g to the jaw.<br />

The ECG shows ST segment elevation <strong>in</strong> II, III and aVF, with T-wave<br />

<strong>in</strong>version <strong>in</strong> V5 and V6.<br />

Anterolateral MI would <strong>in</strong>volve leads related to that portion of the heart,<br />

e.g. V4–V6, I, aVL.<br />

ECG of a 55 year old be<strong>in</strong>g treated for hypertension shows tall tented<br />

T waves.<br />

Look out for potassium-spar<strong>in</strong>g diuretics and angiotens<strong>in</strong>-convert<strong>in</strong>g<br />

enzyme (ACE) <strong>in</strong>hibitors as a cause. Other causes <strong>in</strong>clude renal failure<br />

and metabolic acidosis.<br />

A 34-year-old man presents to A&E after a road traffic accident. The<br />

ECG shows pulseless electrical activity.<br />

Pulseless electrical activity (PEA) is a cl<strong>in</strong>ical condition characterized by<br />

loss of a palpable pulse <strong>in</strong> the presence of recordable cardiac electrical
