A 60-year-old woman presents with constant backache. Her ESR and<br />
serum calcium are markedly elevated.<br />
Back pa<strong>in</strong> is common <strong>in</strong> older people and the cause is often benign.<br />
However, constant symptoms, raised ESR and raised serum calcium raise<br />
the suspicion of underly<strong>in</strong>g myeloma.<br />
A 65-year-old man with osteoarthritis compla<strong>in</strong>s of back pa<strong>in</strong> worse on<br />
walk<strong>in</strong>g along, with ach<strong>in</strong>g and heav<strong>in</strong>ess <strong>in</strong> both legs that force him to<br />
stop walk<strong>in</strong>g. Pa<strong>in</strong> is relieved slowly after rest or lean<strong>in</strong>g forward.<br />
This is a classic description of sp<strong>in</strong>al claudication (lean<strong>in</strong>g forward opens<br />
the sp<strong>in</strong>al canal and relieves pa<strong>in</strong>). In vascular claudication the pa<strong>in</strong> is<br />
quickly relieved on rest, whereas the relief from pa<strong>in</strong> occurs later <strong>in</strong><br />
sp<strong>in</strong>al claudication. Operative decompression can be very successful <strong>in</strong><br />
treat<strong>in</strong>g symptoms of pa<strong>in</strong> <strong>in</strong> patients who are severely affected.<br />
A 13-year-old girl compla<strong>in</strong>s of backache and fatigue. She is worried<br />
that she is becom<strong>in</strong>g <strong>in</strong>creas<strong>in</strong>gly round-shouldered. On exam<strong>in</strong>ation,<br />
she has a smooth thoracic kyphosis. Her radiograph shows wedgeshaped<br />
vertebral bodies <strong>in</strong> the thoracic sp<strong>in</strong>e.<br />
Scheuermann’s disease is a rare condition that typically affects females<br />
dur<strong>in</strong>g puberty. There is abnormal ossification of r<strong>in</strong>g epiphyses of thoracic<br />
vertebrae. The deform<strong>in</strong>g forces are greatest at the anterior border<br />
of the vertebrae giv<strong>in</strong>g rise to the described wedge-shaped vertebral bodies<br />
(i.e. narrower anteriorly). Management depends on the degree of<br />
kyphosis and ranges from back-strengthen<strong>in</strong>g exercises and postural<br />
tra<strong>in</strong><strong>in</strong>g <strong>in</strong> those mildly affected to operative correction and fusion <strong>in</strong><br />
those with severe kyphosis.<br />
A 35-year-old tourist compla<strong>in</strong>s of severe back pa<strong>in</strong> with <strong>in</strong>ability to<br />
straighten up after lift<strong>in</strong>g a suitcase yesterday. He now presents with<br />
weakness of his big toe extension and loss of sensation on the outer<br />
side of the calf.<br />
He is show<strong>in</strong>g symptoms of compression at the L5 level.<br />
