Use DA Label 116 (Signature or Initials Marker) when more than one action on a case is to be signed or when initials<br />

are required on copies other than the record copy. Clip the marker to the signature page. Place the left edge of the<br />

marker flush with the left edge of the paper so that the caption “signature” or “initials” extends beyond the right edge<br />

of the paper. Do not staple the marker to plain bond paper.<br />

Use DA Form 2445 (<strong>Correspondence</strong> Control Record) to control a piece of correspondence, mail, or a message by<br />

assigning action to a specific organization or individual <strong>and</strong> specifying a completion date. Retain a copy in suspense<br />

until the completed action is taken.<br />

Use the OF 27 (Two-Way Memo) for interoffice messages, notes, <strong>and</strong> short replies. Instructions for completing the<br />

form are on the form.<br />

7–10. DA Form 200 (Transmittal Record)<br />

a. Use the DA Form 200 (Transmittal Record) when providing an addressee information that is not contained in the<br />

document being sent.<br />

b. Do not use the transmittal record—<br />

(1) To transmit pamphlets, instruction booklets, or other publications that are self-explanatory.<br />

(2) To forward a form or report when its design provides for inserting the addressee, originator, <strong>and</strong> a submission<br />

date.<br />

8–1. General authority<br />

This chapter, an extract of DOD 5200.1–PH, DOD Guide to Marking Classified Documents, contains illustrations <strong>and</strong><br />

sample classified memor<strong>and</strong>ums to show proper security classification marking of correspondence. The text in the<br />

illustrations (figs 8–1 through 8–9) covers most of the important information. This chapter does not—<br />

a. Contain or reveal classified information. Markings are for illustration only.<br />

b. Change or repeat DOD requirements in AR 380–5. Refer to AR 380–5 when marking correspondence.<br />

c. Illustrate every conceivable situation that may be encountered in the production of classified correspondence.<br />
