Please enter the specific speed limit <strong>for</strong> the case vehicle if it is different from stated speed<br />

limit. The limits <strong>for</strong> goods vehicles, buses, coaches <strong>and</strong> towing vehicles are generally<br />

restricted on most roads with speed limits over 80Kph<br />

Any defect which is reported in the incident documentation which had a direct impact on the<br />

<strong>causation</strong> <strong>of</strong> the incident, <strong>for</strong> example a tyre or suspension failure in a bend or an electronic<br />

failure <strong>for</strong> ABS or other active safety system.<br />

Suspected defects related to <strong>causation</strong> or any other failure <strong>of</strong> safety systems/vehicle<br />

components whether related to the <strong>causation</strong> or not should be reported in the comments<br />

box<br />

Choose “No” if vehicle is overdue <strong>for</strong> an inspection.<br />

If the vehicle has not had an inspection, but does not need one (because it is less than 3<br />

years old) code as Not applicable.<br />

This applies to all vehicles (except pedestrians) <strong>and</strong> is coded <strong>for</strong> each vehicles direction <strong>of</strong><br />

travel. The coding should best describe the very last manoeuvre <strong>and</strong> as such should reflect<br />

the location/orientation <strong>of</strong> the <strong>accident</strong><br />

NB. Please describe in the comments box what the distraction was, e.g. animal in roadway,<br />

advertising, children inside car etc.<br />
