Francis Barko was so seriously injured August 3d by a fall of<br />

slate that death resulted two day® afterwards. The accident occurred<br />

at face of room where he was engaged at his regular work.<br />

Samuel Cook was so seriously injured on August 6th by a fall<br />

slate thai death resulted four days alter. Cook was in a stooping<br />

posit ion and engaged in undercutting coal when the slate fell. His<br />

brother was at work with him at the time and stated that they had<br />

tried to take the slate down a whorl time before the accident occurred,<br />

but could not.<br />

Angelo Vallanna was instantly killed August 13th by a fall of<br />

roof. The accident occurred at face of room pillar 11, off 7 Butt<br />

entry lower level, while he was engaged in mining out a small stump<br />

of coal which had been left to assist in supporting the roof, until<br />

he was ready to<br />

draw the timber which he was preparing to do at<br />

the time of the accident.<br />

Andy Okula while at work in No. 1 "A" Southwest mine, was instantly<br />

killed on August 13th by a fall of roof at face of pillar<br />

workings.<br />

Thomas Stevenson, an oiler at St. Clair mine, was fatally injured<br />

on August '2 1st by his skull having been crushed between two mine<br />

wagons; death resulted six hours after.<br />

This accident occurred outside of the mine and near the foot of<br />

tipple, where he was engaged in oiling mine cars, also in assisting<br />

to couple and uncouple the trips. A trip of several wagons had<br />

been pulled to the foot of the tipple, and as only six or seven are<br />

hoisted upon the tipple at one time, it was necessary that this<br />

number be cut off. Stevenson was standing on the inside of curve<br />

when the trip was stopped for the purpose of cutting off the regular<br />

number for the tipple trip. While reaching in between two of the<br />

wagons to remove the coupling, by some means the wagons in front,<br />

which were standing on a grade, moved back and his head was<br />

caught between them.<br />

William Campfield was fatally injured September 10th by a fall<br />

of slate, and death resulted in three hours. The accident occurred<br />

at face of room in which he was working.<br />

William Burns was instantly killed September 29th by a fall of<br />

slate at face of entry pillar, where he was at work.<br />

Ge<strong>org</strong>e W. Altman was fatally injured on October 4th by a fall<br />

of coal at face of room, ami death resulted while he was being taken<br />

home.<br />

John Shedlock was instantly killed October LOth by a fall of roof.<br />
